静脉和硬膜外双路预给微量舒芬太尼预防阑尾牵拉反应效果观察

摘要 目的 探讨静脉和硬膜外双路预给微量舒芬太尼预防阑尾牵拉反应的效果.方法 将120例行阑尾切除手术患者按自然数随机排列法分为四组:A组(静脉和硬膜外双路预给舒芬太尼组)、B组(静脉预给舒芬太尼组)、C组[静脉预给氟哌利多、芬太尼复合液(氟芬合剂)组]和D组(术中给药组),每组各30例,观察记录各组患者术中阑尾牵拉反应程度、进腹前和术毕Ramsay评分及不良反应.结果 A、B、C组手术时间均较D组短(P<0.05);Ramsay评分进腹前A、B组低于C组高于D组(P<0.05),术毕A组低于B、C、D组(P<0.05).A组无阑尾牵拉反应率为60.0%(18/30),均高于B组(23.3%,7/30)、C组(26.7%,8/30)、D组(0)(P<0.05).A组低脉搏血氧饱和度、低血压、低心率发生率均低于B、C、D组(P<0.05).结论 静脉和硬膜外双路预给微量舒芬太尼,类似于超前镇痛和多模式镇痛理论,抑制阑尾牵拉反应,效果满意.
