摘要
目的探讨跗外侧动脉皮瓣移植修复手足部皮肤缺损的临床效果。方法在足背外侧设计以足背动脉为蒂的跗外侧动脉皮支穿出点为中心的逆行皮瓣修复手足部皮肤缺损,皮瓣的足背外侧皮神经在足部与跖底神经吻合,手部动脉与桡动脉浅支吻合,神经与桡神经浅支吻合,足外侧供区用腹股沟全厚皮打包植皮。结果手术时间:带蒂皮瓣转移85~240min,平均160min;游离皮瓣185~425min,平均430min。术后51例皮瓣全部成活。35例随访8~36个月,平均19个月,跗外侧动脉皮瓣及植皮全部成活,感觉、外形良好,不需要二期整形。结论跗外侧动脉皮瓣逆行转移修复手足部皮肤缺损,皮瓣薄、有神经支配、解剖位置恒定。
Objective To explore the efficacy of lateral tarsal artery flap implantation in the treatment of hand and foot cutaneous defect.Methods Cutaneous defect on hand or foot was covered by a retrograde flap which pedicled on the dorsal pedal artery and centered at perforating site of the cutaneous branch of lateral tarsal artery.The accessory lateral dorsal cutaneous nerve of the foot was inosculated with plantar metatarsal nerve in repairing defect on foot,or with superficial radial nerve in repairing defect on hand.Donor site defect on lateral foot was covered with full thickness skin from fold inguen by dermatoplasty.Results The mean operation time was 160 min(85-240 min)for transplanting pedicled flap and 430 min(185-425 min)for seperating the flap.The flap survived after the operation in all of the 51 patients.Follow-up was achieved in 35 patients for 8 to 36 months with a mean of 19 months,during which both lateral tarsal artery flap and transplanted skin survived with satisfying sensory and profile without needing subsequent reshaping.Conclusion In cutaneous defect coverage,neurosensory lateral tarsal artery flap is recommended for its advantages in thinness,nerve innervated,and constant anatomical position.
