海南省2008年手足口病样本核酸检测分析 被引量:6

Viral nucleic acid detection with RT-PCR for specimens from hand-foot-mouth disease cases in Hainan province,2008

摘要 目的了解2008年海南省手足口病(HFMD)暴发的病原及主要的病毒型别,为HFMD病例诊断和防控措施提供科学依据。方法依据卫生部《手足口病预防控制指南》提取HFMD临床诊断病例各种样本中的病毒核酸,用肠道病毒通用引物、肠道病毒71型(EV71)和柯萨奇病毒A16型(CoxA16)特异性引物进行逆转录-聚合酶链式反应(RT-PCR),检测基因片段大小。结果 274例临床诊断手足口病病例中EV71型核酸阳性、柯萨奇病毒A16核酸阳性、其他型别肠道病毒核酸阳性分别有102,12,35例;样本中疱疹液阳性检出率最高,为72.0%,肛拭子为40.0%,鼻咽拭子为34.8%,其余样本均阴性;54份直接提取病毒核酸进行RT-PCR检测,结果阴性但细胞培养出现细胞病理改变(CPE)的分离物;肠道病毒、EV71型、CoxA16型阳性分别有48,29,4份,阳性率分别为88.89%,53.70%,7.40%;其中1份样本EV71型和CoxAl6型均为阳性。结论引起海南省2008年手足口病暴发流行的主要病原是肠道病毒71型,其次为CoxAl6型。 Objective To reveal the pathogen of hand-foot-mouth disease(HFMD) and to know the main virus type in an outbreak of HFMD in Hainan province in 2008.Methods According to the "Hand-foot-mouth disease control and prevention guide(2008)" issued by Health Ministry,the nucleic acids from specimens such as nasal and throat swabs,anus swabs,herpes fluid of clinical diagnosed cases of HFMD were isolated.The RNA was amplified by reverse transcription-polymerase chain reaction(RT-PCR) using enterovirus universal primer,enterovirus type 71 primer and coxsackie virus group A type 16 primer.Results Among 274 clinically diagnosed cases,EV71 virus RNA was positive in 102 cases,CoxA16 virus RNA in 12 cases,and other enterovirus RNA in 35 cases.The RNA testing positive rate of the herpes fluid was 72.0%,with a positive rate of 40% for anus swabs and 34.8% for nasal and throat swabs.The RNA of 54 specimens detected directly by RT-PCR was negative.But these specimens inoculated to RD cell line showed cytopathic effect and were identified by RT-PCR.The results showed that enterovirus RNA was positive in 48 specimens(88.89%).EV71 RNA was positive in 29 specimens(53.7%);Cox A16 RNA was positive in 4 specimens(7.4%).Both EV71 RNA and CoxA16 RNA were positive in 1 specimen.Conclusion The major pathogen of HFMD was EV71 causing the outbreak in Hainan province in 2008.CoxA16 was one of the pathogens.The etiology surveillance of HFMD should be strengthened in order to know the change of virus type and the trend of toxicity change.
