摘要
目的探讨胸脐皮瓣修复多发伤致四肢大面积软组织缺损患者的护理措施。方法 四肢大面积软组织缺损患者15例,围术期护理措施包括伤后注重骨折固定、控制伤口出血、肢体血供的观察及感染的控制、加强术后体位管理、预防并发症和指导早期功能锻炼。结果 15例患者中,创面、骨折愈合,皮瓣成活14例,坏死1例。上肢屈肘功能恢复良好9例,下肢行走及负重功能恢复6例。结论加强基础护理、术后体位管理、观察皮瓣血供、预防并发症、肢体功能锻炼是是胸脐皮瓣手术成功的重要因素。
Objective To summarize the nursing for the repair of multiple injury-caused large area soft tissue defect on the limbs by thoraco-umbilical flap transplantation.Methods The repair of large area soft tissue defect on the limbs by thoraco-umbilical flap transplantation was performed in 15 cases.The treatments in nursing included fracture fixation,control of wound blooding,careful observation of the limbs and flap blood supply,prevention of infection and early functional exercise teaching.Results Of 15 cases,14 flaps were survived with complete wound healing and fracture union.One flap was necrosed.Functions of flexor of the elbow were recovered in 9 upper limbs.Walking function and weight loading regained in 6 lower limbs.Conclusion Thoraco-umbilical flap is ideal for repair of multiple injury-caused large area soft tissue defect on the limbs.The important factors for successful repair of multiple injury-caused large area soft tissue defect are basic nursing,postural management,observation of flap blood supply,prevention of complication and early functional exercise.
