法洛四联症并小左心室行体肺分流术效果分析 被引量：1

摘要 目的 探讨法洛四联症并小左心室应用Groe-Tex人造血管行体肺分流术的手术效果. 方法12例法洛四联症并小左心室患者应用Gore-Tex人造血管行分流术,中心分流术3例,周围肺动脉分流术9例,均在非体外循环下手术.Gore-Tex人造血管直径4～7 mm.术后5～18个月测量左心室舒张末期容积指数（LVEDVI）、血氧饱和度和左、右肺动脉直径之和与膈肌水平降主动脉直径比值（McGoon值）,分流术前后相关数据采用配对t检验进行统计分析.结果 全组无早期手术死亡,1例术后4个月死于肺炎.术后经皮血氧饱和度由（76.8±9.0）%增加到（87.5±2.4）%,差异有统计学意义（P〈0.01）;McGoon比值由1.42±0.27增加到1.62±0.32,差异有统计学意义（P〈0.05）;LVEDVl由（20.1±7.0）ml/m2增加到（34.3±7.4）ml/m2,差异有统计学意义（P〈0.01）.10例患者在术后6～18个月内接受二期根治手术,无死亡病例.1例患者缺氧症状改善,等待根治手术.结论 应用合适口径Gore-Tex人造血管行体肺分流术可明显增加法洛四联症并小左心室患者的左心室容积指数,为左心室发育小的患者行根治创造良好的手术条件.
