摘要
目的探讨有限元分析方法在颞下颌关节强直术前计划中的应用价值。方法将1例颞下颌关节真性强直患者的CT图像数据导入Mimics13.1软件进行三维重建,并在模型上虚拟两种不同的手术方式(钢板固定和非钢板固定),通过有限元分析Abaqus软件建立正常下颌骨及不同虚拟手术的下颌骨有限元模型,定量分析正常下颌骨和颞下颌关节强直患者的髁突应力分布状况。结果未采用钢板固定的虚拟手术模型其髁突应力分布值比正常下颌骨为大,而采用钢板固定虚拟手术者的髁突应力分布值与正常人接近。结论颞下颌关节强直手术可以运用有限元分析方法建立有限元模型并进行模拟手术,对颞下颌关节强直治疗方法的选择具有指导性作用。
Objective The aim of this study is to evaluate the applicable value of finite element analysis (FEA) for presurgical planning in the treatment of temporomandibular joint (TMJ) ankylosis. Methods CT image data of one patient with unilateral TMJ ankylosis were imported to Mimics 13.1 software and then reconstructed into 3D mandible model. Two surgical methods (plate fixation and non-plate fixation) were simulated. Normal and TMJ mandible finite element models under two different virtual surgical methods were established subsequently through FEA Abaqus software. Conditions of condylar process stress distribution of normal mandible and TMJ ankylosis mandible under two surgical procedures were analyzed quantitatively. Results The value of condylar process stress distribution of mandible under non-plate fixation virtual surgery was larger than normal value, but that of mandible under plate fixation virtual surgery was near to normal value. Conclusion For TMJ ankylosis surgery, EFA technique can be used to construct finite element model and then to mimic virtual surgery therefore provide information for doctors to choose surgical methods.
