摘要
目的应用3.0T三维磁共振波谱(3D MRS)研究前列腺外周带T2低信号的代谢变化特点,并探讨其对外周带病变的诊断价值。方法回顾性分析经手术病理或穿刺活检证实的26例前列腺疾病患者,分为前列腺癌和前列腺良性病变2组:前者12例;后者14例,其中包括前列腺增生10例,前列腺炎4例。依据T2低信号区设定外周带感兴趣区,利用多体素三维磁共振波谱分析(3D MRS)测定各兴趣区(胆碱+肌酸)/枸橼酸盐(Cho+Cre)/Cit的比值,并比较两组(Cho+Cre)/Cit比值的差异。结果所有患者均成功完成检查。前列腺癌组(Cho+Cre)/Cit平均值为1.95±0.45,前列腺良性病变组(Cho+Cre)/Cit平均值为0.69±0.21;两组比较差异有统计学意义(P<0.05)。结论 3D MRS能定量评价外周带T2信号减低的代谢变化,对前列腺良恶性病变具有重要的诊断价值。
Objective To evaluate the diagnosis of prostate gland disease with low T2WI signal in peripheral zone of prostate by 3.0 Tesla three-dimensional magnetic resonance spectroscopy (3D MRS). Methods 26 cases with prostate gland disease proven by surgery and biopsy were retrospectively analyzed. All cases were divided into two parts: malignant prostate group and benign group, the former group includes 12 cases, and the latter group includes 10 cases of patients with prostatic hyperplasia and 4 cases of patients with prostatitis. The (Cho+Cre)/Cit ratio was ascertained and compared in the low T2WI signal area with 3D MRS. Results All the patients successfully completed inspection. The Average value of (Cho + Cre) /Cit in prostate cancer group is 1.95 0.45. In the benign prostatic disease group the average value is 0.69 0.21.There is statistically significant difference between the two groups(P0.01). Conclusions The study demonstrates that MRS can be reliably acquired using a body coil receiver. MRS may also play a role in differentiating benign prostate disease from the prostate cancer.
