摘要
目的总结手指电烧伤的修复特点。方法分析了83例269个手指电烧伤,对伤情特点及修复方式进行了探讨。结果手指电烧伤具有多发性的特点,决定了创面修复上的复杂性,手术时机应根据不同情况而定,一般情况下可早期手术,但近环形或环形创面,早期只宜作减张手术或单纯焦痂切除,避免彻底扩创损伤残存血管,待手指远端血运充分改善后,再行延迟手术修复创面,这样可保留手指,效果良好。结论手指电烧伤创面修复可采用多种皮瓣,选用皮瓣的顺序应依次为同指皮瓣、同手皮瓣、远位皮瓣。
Objective To summarize the clinical feature of the fingers with electrical burns.Meth- ods 269 fingers with electrical burns were analysed in the characteristics of injury and the types of repair. Results Fingers with electrical burns were often multiple,characterizing the complexity of the wound re- pair.Operation time should be determined by different conditions.Usually the fingers with electrical burns should be operated early but the circunferential wound surface should be depressed or eschar excised during early stage in order to avoid ischemia of distal segment.When the circulation of the finger distal to the in- jury is improved,operation should be done to repair wound surface.Conclusion Many kinds of flaps might be used for repair of fingers with electrical burns.The flaps from the same finger should be the first choice, followed by flaps from the same hand and distant flaps.
