急性一氧化碳中毒性心肌损害的临床分析 被引量:2

摘要 目的:探讨急性一氧化碳中毒对心肌的损害情况。方法:采用回顾性调查方法,分析132例急性。一氧化碳中毒患者,按照中毒程度分为轻度、中度、重度3组,分析其血清心肌酶、心肌肌钙蛋白I(cTnI)的变化及差异和心电图改变。结果:血清心肌酶、心肌肌钙蛋白I在急性期变化明显,中、重度中毒组较轻度中毒组心肌酶和cTnI水平明显升高,组间差异有显著意义(p<0.01)。有心电图改变者59例,占44.7%,以ST-T改变和窦速为主。结论:急性中、重度一氧化碳中毒患者有较高的心肌损害发生率,中毒越重,心肌酶、cTnI升高越显著,心电图ST-T改变越明显,心肌损害越明显。 Objective:To approach the condition of myocardial damage resulting from acute carbon monoxide poisoning (ACOP). Methods:To divide 132 cases of ACOP according to the degree of intoxication into 3 groups of light,medium and severe ones.Then,to analyse the change of myocardial enzyme, cardiac troponin I(cTnI)and electrocardiogram(ECG) useing retrospective study.Results:The myocardial enzyme and cTnl changed obviously in acute stage.Myocardial enzyme and cTnI level increased significantly in medium and severe intoxication groups compared with light one (P〈O.Ol).There were ECG changes 0f59 cases,accounting for 44.7%,most of changes were ST-T alteration and sinus taehycardia.Conelusion:The patients with acute medium to severe degree carbon monoxide poisoning had a higher incidence of myocardial damage.The more serious the poisoning,the more significantly the myocardial enzymes and cTnl increased,the more obviously the ECG ST-T changed,the more obvious the myocardial damage.
