摘要
目的评价改良吻合器痔上黏膜环切术(PPH术)在治疗Ⅲ、Ⅳ期内痔的临床效果。方法将90例Ⅲ、Ⅳ期内痔患者随机分为痔环形切除术组、常规PPH组和改良PPH手术组,每组各30例,比较观察3组的手术时间、术后疼痛、术后并发症及术后复发情况。结果改良PPH组与常规PPH组比较,手术时间、术后疼痛均无显著性差异(P均>0.05),改良PPH组术后并发症发生率、复发率均低(P均<0.05)。痔环形切除术组与其他2组比较手术时间、术后疼痛、术后并发症发生率、复发率均有显著性差异(P均<0.05)。结论改良PPH在Ⅲ、Ⅳ期内痔治疗中能明显减少手术时间、术后疼痛,降低术后并发症发生率及复发率,改良PPH是治疗重度脱垂性内痔的理想方法。
Objective It is to evaluate the clinical effect of improved procedure for prolapse and hemorrhoids（PPH） on internal hemorrhoid in Ⅲ and Ⅳ stage.Methods 90 patients with internal hemorrhoid in Ⅲ and Ⅳ stage were randomly divided into whitehead operation group,routine PPH group and improved PPH group in which there was 30 cases.The status of operation time,postoperative pain,postoperative complications and postoperative recurrence in the three groups were observed and compared.Results There were both no significant differences on operation time and postoperative pain between improved PPH group and routine PPH group（both P0.05）.The rates of postoperative complications and postoperative recurrence in improved PPH group were both lower than that in routine PPH group（both P0.05）.There were all significant differences on operation time,postoperative pain and the rates of postoperative complications and postoperative recurrence between whitehead operation group and the other two groups（all P0.05）.Conclusion Improved PPH can obviously decrease operation time and postoperative pain and lower the rates of postoperative complications and postoperative recurrence in the treatment for internal hemorrhoid in Ⅲ and Ⅳ stage.Improved PPH is an ideal treatment method for severe Prolapsed internal hemorrhoids.
