肩关节后脱位的损伤机理及影像学检查 被引量:3

INVESTIGATION THE PATHOGENESIS OF SHOULDER POSTERIOR DISLOCATION AND IMAGING EXAMINATIONAL TECHNOLOGY USED FOR DIAGNOSIS

摘要 目的探讨肩关节后脱位的病因及X线平片与CT扫描在肩关节后脱位中的应用价值。方法分析25例肩关节后脱位患者的临床资料及其X线平片、CT检查的显示情况。结果 25例肩关节后脱位的X线检查正位片显示肱骨轻度外展,肱骨头呈功能位或内旋位,肱骨大结节旋前,关节间隙仍然存在,"泪滴"影变小;侧位片显示肱骨头向后侧脱位;腋窝位片显示肱骨头位于关节盂后方。CT检查显示为肱骨头向后侧旋转,脱出于关节盂后缘。结论肩关节后脱位结合临床X线和CT检查能够作出正确的诊断。 Objective To study the pathogenesis of shoulder posterior dislocation and the importance of using spiral CT and X - ray plain films in diagonosis. Methods Analyze the CT examination and X - ray plain films signs of 25 cases after the shoulder posterior dislocation with patient' s clinical data. Results X - ray film of 25 eases shoulder posterior dislocation showing the performance of the humerus slightly abducted, its head is situated in functional place or internal rotation, its large nodule turn pronation, no abnormal joint space, "tear drop" shadow smaller; the lateral film shows humeral head shift backward; axillary fossa film shows humeral head sited behind glenoid cavity. CT examination shows humeral head rotate backward emerged from posterior edge of glenoid cavity. Conclusion CT and X - film associated clinical manifestation can make correct diagnosis of shoulder posterior dislocation.
