摘要
目的研究海绵窦区病变的病因、发生机制、临床特点、影像学变化及诊断。方法回顾性分析20例确诊的海绵窦区病变患者的临床、生化、病理及影像学检查,并结合文献进行分析。结果 20例海绵窦区病变患者中,13例(65.0%)为Tolosa-Hunt综合征,另7例(35.0%)由脑膜癌、鼻咽癌、血栓性海绵窦炎、颈内动脉海绵窦瘘、结核性肉芽肿侵犯海绵窦区所造成。腰穿、头部MR和DSA检查有助于明确诊断。结论海绵窦区病变可由不同的病因引起,明确它们之间的鉴别诊断,有利于提高其正确的治疗。
Objective To investigate the etiology,diagnosis,clinical feature,image of diseases of carvernous sinus zone.Methods Clinical feature,test,pathology and image of the 20 cases with diseases of carvernous sinus zone were reviewed retrospectively.Results Among 20 patients,the most common cause was Tolosa-Hunt syndrome （65.0%）.Meningeal carcinoma,carotid fistulas,tuberculous granuloma could all be the cause of diseases of carvernous sinus zone.Lumbar puncture,head MR and DSA could help the diagnosis.Conclusion Diseases of carvernous sinus zone can be caused by different diseases.Distinguishing them can help to the further correct therapy.
