摘要
用放免法检测61例肝硬化患者与正常对照组的血浆内皮素-1(ET-1)和肿瘤坏死因子-α(TNF-α)。结果:肝硬化组血浆ET-1及TNF-α水平显著高于对照组(P均<0.001),且在肝功能Child-Pugh分级中,呈现C>B>A级的规律。组间分析表明肝硬化患者食管静脉曲张伴大中量腹水组(LC4)及仅有大中量腹水组(LC3)的血浆ET-1和TNF-α均显著高于食管静脉曲张伴少量腹水或无腹水组(LC2),LC2组高于无腹水和亦无静脉曲张组(LC1)(P均<0.05)。LC1组及Child-PughA级患者与对照组间无明显差别(P>0.05)。相关分析显示肝硬化者血浆ET-1和TNF-α间存在明显正相关。认为肝硬化者血浆ET-1和TNF-α水平可反映肝硬化的严重程度。
Endothelin 1(ET 1) and tumor necrosis factor α(TNF α) were measured in plasma samples collected from 24 healthy controls and 61 liver cirrhosis patients.Results:Plasma ET 1 and TNF α of cirrhotic patients were significantly higher than those of healthy controls(P<0 001). Comparisons among the level of plasma ET 1 and TNF α classified by liver function were shown as follows:Child Pugh C>B>A(P<0 05).Analysis among groups suggested plasma ET 1 and TNF α were markedly higher in both groups with esophageal varices accompanied by severe or moderate ascites(LC 4) and with simple severe or moderate ascites(LC 3) than in group with esophageal varices accompanied by mild or no ascites(LC 2),which was also significantly higher in group LC 2 than in group without varices and ascites(LC 1)(P<0 05).No statistical difference of plasma ET 1 and TNF α was found between group LC 1 or Child A and normal control, so was between group LC 4 and group LC 3(P<0 05).There was positive correlation between plasma ET 1 and TNF α.Conclusion:The increases of plasma ET 1 and TNF α are closely associated with the severity of liver cirrhosis and directly or indirectly contribute to the formation and progression of portal hypertension.
