摘要
目的探讨脐血间质干细胞对帕金森病(PD)患者的临床症状、基底节区脑血流灌注量影响及应用脐血间质干细胞移植后的安全性。方法 20例PD患者入院后随机分为脐血间质干细胞移植组及常规治疗组,脐血间质干细胞移植组患者在常规治疗的基础上应用鞘内及静脉移植脐血间质干细胞,两组患者于治疗前、治疗后1、2、3和6月进行PD综合评分量表(UPDRSⅢ)评分、Hoehn-Yahr分级判定以及以单光子发射计算机断层成像术(SPECT)检测,观察其疗效及不良反应。结果应用脐血间质干细胞移植的PD患者治疗后1、2和3月的UPDRSⅢ评分、Hoehn-Yahr分级均优于常规治疗组的PD患者,治疗后3月明显,两组比较有统计学意义(P<0.01),治疗后6月UPDRSⅢ评分、Hoehn-Yahr分级与治疗前及常规治疗组比较无统计学意义(P>0.05);SPECT检测结果显示脐血间质干细胞移植组PD患者治疗后2月、3月基底节区脑血流灌注量增加,治疗后3月尤为明显,SPECT放射性计数比值与常规治疗组比较有统计学意义(P<0.01)。结论脐血间质干细胞移植对改善PD临床症状、基底节区脑血流灌注量、延缓病程及提高日常生活质量疗效显著,不良反应轻,但疗效不持久。
Aim To observe the effect of umbilical cord blood mesenchymal stem cells(MSCs) transplantation on Parkinson Disease patients.Methods A total of 20 Parkinson disease patients were divided into two groups,10 patients received MSCs transplantation by lumbar puncture via subarachnoid space.One,2,3 and 6 months after treatment Unified Parkinson's Disease Rating Scale Ⅲ(UPDRSⅢ),Hoehn-Yahr class ification and Single-photon emission computed tomography(SPECT)were used to evaluate neural function and cerebral blood flow of bilateral basal ganglia of Parkinsom disease patients treated.Results Compared to that before transplantation,the score of UPDRSⅢ was significant decreased espacialy in those 3 month after treatmen(tP 0.01).The clinical symptoms,such as tremor,rigidity,slow movement,unstable posture were obvious improveed.Besides the grading of Hoehn-Yahr classification.cerebral blood flow of bilateral basal ganglia in those 3 month after treatment was better than those before treatmen(tP0.01).The blood flow in the patients one,2 and 3 months after transplantation was increased as compared to the normal controls as showed by SPECT.In addition,there was no graft versus host disease occurred.Conclusion MSCs transplantation can ameliorate clinical symptoms,improve life quality of Parkinson disease patients to some extent,increase cerebral blood flow of bilateral basal ganglia.
