摘要
目的:探讨血小板膜糖蛋白Ⅱb和Ⅲa在糖尿病合并急性心脑血管病中的变化及其临床意义。方法:检测24例糖尿病合并急性心脑血管病患者及77例非糖尿病合并急性心脑血管病患者急性发病后2周内血小板膜糖蛋白Ⅱb和Ⅲa的含量变化,并与30例正常人对照。结果:急性心脑血管病患者血小板膜糖蛋白Ⅱb和Ⅲa均显著高于正常对照组(P均<0.01),伴糖尿病者发病72小时内血小板膜糖蛋白Ⅱb和Ⅲa均显著高于非糖尿病者(P均<0.05),随后渐下降;77例非糖尿病急性心脑血管病患者发病后21例有一过性应激性血糖升高〔(7.47±0.83)mmol/L比(5.53±0.47)mmol/L,P<0.01〕,但血小板膜糖蛋白Ⅱb和Ⅲa却无显著差异;发病时糖尿病组与非糖尿病组血小板、红细胞及白细胞计数均无明显变化。结论:急性心脑血管病患者均处于血栓前状态,伴有糖尿病患者尤为严重;血小板膜糖蛋白Ⅱb和Ⅲa含量与血小板数量无关。
Objective:To observe the clinical significance of changes in platelet membrane glycoprotein (PG) Ⅱb and Ⅲa in diabetic patients with acute ischemic cardiac or cerebral vascular diseases.Methods:Serum PG Ⅱb and Ⅲa concentrations were measured in 24 diabetic and 77 nondiabetic cases with acute ischemic cadiac or cerebral vascular diseases during two weeks,and 30 healthy volunteer served as controls.Results:PG Ⅱb and Ⅲa concentrations in patients with acute ischemic cardiac or cerebral vascular diseases were significantly higher than that in control group (all P <0 01),and furthermore,they were significantly higher than those in nondiabetic patients within 72 hours (all P <0 05).Among 77 nondiabetic cases 21 cases had sustained stress hyperglycemia after the episode of acute ischemic cardiac or cerebral vascular diseases,and blood sugar levels were significantly higher than those without stress hyperplycemia (7 47±0 83)mmol/L vs.(5 53±0 47)mmol/L, P <0 01 .But there were not any differences in PG Ⅱb and Ⅲa and in platelet,red blood cell and white blood cell counts at the onset of acute ischemic cardiac or cerebral vascular diseases.Conclusions:All the patients with acute ischemic cardiac or cerebral vascular diseases are in a prethrombosis condition,particularly in those with diabetes.It appears no marked correlation between PG Ⅱb and Ⅲa and platelet counts,sole hyperglycemia is not the main factor associated with elevation of PG Ⅱb and Ⅲa.
