子宫腺肌症患者子宫内膜-肌层界面雌激素受体变化 被引量:6

Changes of estrogen receptor in endometrial myometrial interface of patients with adenomyosis

摘要 目的:探讨子宫腺肌症患者在位子宫内膜、子宫内膜-肌层界面(EMI)增生期雌激素受体变化及其对腺肌症发病的影响。方法:收集正常妇女子宫内膜(及部分肌层)40例和腺肌症患者在位内膜(及部分肌层)50例,采用免疫组化方法测定、分析雌激素受体(ERa)的变化。结果:①子宫腺肌症组ERa阳性的腺上皮细胞数量在增生早期为86.7%,正常内膜组为50.0%,差别有统计学意义;且腺上皮细胞胞质中也可见ERa阳性颗粒。子宫腺肌症组和正常内膜组在增生中期、增生晚期ERa阳性细胞数量差异无统计学意义。②子宫腺肌症组EMI上ERa阳性信号显著增强。结论:子宫腺肌症患者子宫内膜中ERa阳性细胞数量增加,EMI在近肌层处方向性生长位点ERa的信号强度较正常内膜组明显增强;相对于正常者的改变,雌激素效应增强,可能是子宫腺肌症患者子宫内膜发生浸润性生长的分子学基础。 Objective: To explore the changes of estrogen receptor ( ER) in proliferative phase of eutopic endometrium and endometrial myometrial interface and the effects on occurrence of adenomyosis. Methods: Normal endometrium ( and myometrium) from 40 normal women and eutopic endometrium ( and myometrium) from 50 cases with adenomyosis were collected. Immunohistochemical method was used to detect and analyze the changes of ERα. Results: In adenomyosis group,the proportion of glandular epithelial cells with positive ERα was 86. 7% at early proliferative phase,while in normal endometrium group,the proportion was 50. 0% ,there was significant difference; ERα positive particles were found in cytoplasm of glandular epithelial cells in adenomyosis group; there was no significant difference in the amount of glandular epithelial cells with positive ERα at middle and late proliferative phase between adenomyosis group and normal endometrium group; ERα positive signal in endometrial myometrial interface of adenomyosis group enhanced significantly. Conclusion: The amount of glandular epithelial cells with positive ERα in endometrium of patients with adenomyosis increases,ERα positive signal near myometrium of endometrial myometrial interface is more intensive than that in normal endometrium group; effect of estrogen enhances compared to normal women,which may be a molecular basis for invasive growth of endometrium in patients with adenomyosis.
