摘要
目的探讨男性糖尿病足患者骨密度的变化及影响因素。方法比较23例男性糖尿病足组、25例正常对照组一般情况及生化指标,包括病程、体重指数(BMI)、糖化血红蛋白(HbAlc)、空腹血糖(FBG)、甘油三脂(TG)、胆固醇(TC)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C),并采用美国Norland双光能X线骨密度仪测定两组的L2-L4和Neck、Ward三角区、Troch的骨密度,对两组结果进行比较。结果糖尿病足组患者空腹血糖9.06±3.44mmol/L、糖化血红蛋白9.05±1.18、甘油三脂2.49±0.48mmol/L、低密度脂蛋白3.29±0.83mmol/L较正常对照组空腹血糖5.60±1.00mmol/L、糖化血红蛋白4.62±0.68、甘油三脂1.32±0.86mmol/L、低密度脂蛋白2.28±1.06mmol/L显著增高(P<0.05);糖尿病足患者L2-L4和Neck、ward三角区、Troch的骨密度分别为1.01±0.23g/cm2、0.85±0.21g/cm2、0.61±0.16g/cm2、0.72±0.17g/cm2,均低于正常对照组的1.17±0.14g/cm2、1.02±0.06g/cm2、0.76±0.14g/cm2、0.83±0.09g/cm2(P<0.05)。结论糖尿病足病患者更易发生骨质疏松,骨折的危险性远高于正常人群,应将骨密度测量作为糖尿病足病常规检查。
Objective To explore the alteration of bone mineral density in male patients with diabetic foot and the influence factors. Methods The general situation and biochemical parameters, including course of disease, BMI, HbAlc, FBG, TG, TC, HDL-C, and LDL-C, were compared between the diabetic foot group with 23 male patients and the control group with 25 healthy men. Bone mineral densities of L2-L4, Neck, Ward, and Torch area were measured using DXA, and compared between the two groups. Results FBG, HbAlc, TG, LDL-C of male patients with diabetic foot were 9.06 ±3.44 mmol/L, 9.05 ±1.18 mmol/L, 2.49 ±0.48 mmol/L and 3.29 ± 0. 83 retool/L, respectively, which were significantly higher than those in the control group (5.60± 1.00 retool/L, 4.62 ±0.68 retool/L, 1.32 ±0.86 mmol/L, and 2. 28 ± 1.06 mmol/L, respectively, P 〈0. 05). Bone mineral densities of L2-IA, Neck, Ward, and Torch area of marea patients with diabetic foot were 1.01 ±0.23 g/cm^2 , 0.85±0. 21 g/cm^2 , 0.61 ±0. 16 g/cm^2 , and 0.72 ±0.17 g/cm^2 , respectively, which were significantly lower than those in the control group ( 1.17 ± 0. 14 g/ cm^2, 1.02 ± 0.06 g/cm^2, 0.76 ± 0.14 g/cm^2, and 0.83 ± 0.09 g/cm^2, respectively, P 〈 0.05). Conclusion Patients with diabetic foot are vulnerable of having osteoporosis. The fracture risk in these patients is higher than in healthy population. BMD should be used as a routine examination in patients with diabetic foot.
