摘要
脊柱的"三柱学说"是垫枕疗法的解剖学基础,有助于胸腰椎压缩性骨折整复的力学原理建立和适应症的选择。有限元及生物力学研究表明,垫枕使脊柱极度过伸,以关节突为支点,利用患者体重,使前纵韧带紧张,从而恢复椎体高度。垫枕的高度与脊柱的损伤程度相关,垫枕高度的无限增高,不仅影响整复效果,还将导致副损伤,垫枕的极限高度为8-15cm,枕高的低限一般为4-8cm。生物力学的研究表明,垫枕的曲率愈小,牵张力愈大,能更好地使椎体复位,将垫枕的支撑点置于伤椎之后,可以获得最大的复位效应。垫枕时配合水平牵引,可以提高整复效果,但是不适于后柱受损的病例。
The"three-pillar theory"of spine is the foundation of anatomy for pillow therapy, which will help the restitution of thoracic and lumbar vertebral compression fracture of the mechanical principle to establish and indications' choice. Finite element and biomechanical studies have shown that pillow makes the spine extreme hyperextension, as the articulation for a fulcrum, using body weight, so that it can make the ligamenta longitudinale anterius tension, thus restoring vertebral height. Pillow's height relates to the degree of spinal injury ,the pillow's height infinite increased, not only affects the restitution results, will result in side injury, the limit pillow's height is 8-15 cm, t minimum height of pillow is generally 4-8 cm. Biomechanical studies have shown that the smaller the curvature of pillow, the greater the tension force, it can be better able to make vertebra reduction, placing the supporting point of pillow after vertebral, which can achieve the greatest reduction effect. Coordinating the combination headgear when cushion pillow can enhance restoration effect, but this is not suitable for cases of posterior column damage.
