摘要
目的探讨超声心动图诊断胎儿动脉导管早闭或狭窄的价值。方法回顾性分析7例经超声检查并产后证实的胎儿动脉导管早闭或狭窄的超声心动图表现,总结其异常声像图特点。结果动脉导管未显示6例,动脉导管狭窄1例。7例胎儿均右心扩大,肺动脉增宽,三尖瓣轻中度以上反流,反流速度Vmax 2.2～4.3 m.s-1。分娩后即刻超声检查7例胎儿右心缩小,三尖瓣反流减轻,反流速度下降;6例未显示动脉导管,宫内动脉导管狭窄,1例仍残留动脉导管,但6个月后复查动脉导管消失;7例胎儿右心系均恢复正常。结论超声心动图对诊断胎儿动脉导管早闭或狭窄有重要价值。
Objective To explore the value of echocardiography in diagnosis of prenatal closure or stenosis of fetal ductus arteriosus（FDA）.Methods Seven cases with prenatal closure or stenosis of FDA were examined by echocardiography and confirmed postnatally.The echocaodiographic characteristics were reviewed,analyzed.The diagnosis idea was summaried.Results Six cases whose ductus arteriosus were not found,one case whose ductus arteriosus was stenosis.Seven fetal right atrial and right ventricular were expanded,pulmonary artery were widen,tricuspid valve regurgitated over slightly or midrangely,regurgitate velocity Vmax 2.2～4.3 m·s^-1.Foramen ovale valve flow increased.After delivery instantly ultrasonic inspected,right atria,right ventricular and pulmonary arterial were contracted,tricuspid valve regurgitate decreased and velocity was lower,ductus arteriosus were not found in 6 cases.Ductus arteriosus was remained in one case who ductus arteriosus stenosis intrauterine,but ductus arteriosus were not found after 6 months.Seven cases right atrial,ventricular recovery normal.Conclusion Echocardiography have significant value in diagnosis of prenatal closure or stenosis of FDA.
