摘要
目的探讨改进后的手术方式治疗胸中下段食管癌的效果。方法自2006年1月至2009年1月,选择中下段食管癌280例,根据手术方式不同分成2组,每组140例。改进组将胃制成管状从弓后至弓上与食管吻合,对照组将原胃从弓前至弓上与食管吻合,对患者的恢复和并发症发生进行分析比较。结果与对照组相比,改进组患者恢复快,并发症的发生率明显下降,改进组和对照组的肺炎、反流、胃潴留发生率分别为5.0%、32.86%和21.43%、78.57%、4.29%。结论中下段食管癌改进后的手术方式优点多,是值得推行的手术方法。
Objective To investigate the effects of the modified operation in patients with middle and lower thoracic esophageal carcinoma. Methods From Januanry 2006 to Januanry 2009,a total of 280 patients with middle and lower thoracic esophageal carsinoma were taken surgical operations in the department. The patients were divided into 2 groups by the different operation,and there were 140 patients in each group. In the modified group,the gasters were tubulated and then anastomosed to the esophagus from retro-aortic arch to supraaortic arch,and in the control group,the gaster were directly anastomosed to the esophagus from ante-aortic arch to supra-aortic arch. The clinic data of recovery and surgical complications were analyzed and compared between the two groups. Results Compared to the patients in the control group,the patients in modified operation group took less time to recovery and had less rates of surgical complications,such as pneumonia(5. 0% vs 21. 43%) ,back flow(32. 86% vs 78. 57%) and gastric retention(0 vs 4. 29%) . Conclusion The modified operation was better for patients with middle and lower thoracic esophageal carcinoma and should be worthy of extended application.
