密闭式吸痰对气管切开患者痰液菌群的影响 被引量:8

Effect of Closed Suction System on Pathogens Cultured from Patients′ Sputum

摘要 目的对比研究密闭式与开放式吸痰对气管切开行机械通气患者肺部感染菌群的影响。方法回顾性研究综合ICU气管切开行机械通气患者45例,分为密闭式吸痰组(n=22)和开放式吸痰组(n=23);每日监测痰培养结果,分别选取第3、5、7、10、14天痰培养资料进行组间对比研究。结果密闭式吸痰组4个时间点痰培养为单一菌种或培养阴性的患者数明显多于开放式吸痰组(P<0.05或<0.01);而所有5个时间点提示继发新的感染概率分别为(密闭式吸痰比开放式吸痰):第3天0vs8.7%,第5天9.1%vs34.8%,第7天22.7%vs47.8%,第10天27.3%vs56.5%,第14天54.5%vs82.6%,差异有统计学意义(P<0.01)。结论密闭式吸痰可以部分预防继发性肺部感染。 OBJECTIVE To study the effects of closed suction system and open suction system on microbial pathogen cultured from patients′ sputum.METHODS A retrospective study was conducted on 45 patients who received tracheotomy and mechanical ventilation in a general intensive care unit.They were divided into two groups,closed suction system group(CSS,n=22) and open suction system group(OSS,n=23).The sputum samples got from deep part of trachea were cultured.The culture results on the 3rd day,5th day,7th day,10th days and 14th day were got for analysis.RESULTS In CSS group,the number of patients whose pathogens remained unchanged was obviously more than that in OSS group(P〈0.05 or P〈0.01).The percent of new pathogen infection of two groups was obviously different(CSS vs OSS):on the 3rd day 0 vs 8.7%,the 5th day 9.1% vs 34.8%,the 7th day 22.7% vs 47.8%,the 10th day 27.3% vs 56.5%,and the 14th day 54.5% vs 82.6%(P〈0.01).CONCLUSIONS Closed tracheal suction system can partly prevent the secondary infection of lungs.
