摘要
目的：评价酚妥拉明合刺五加注射液治疗肺源性心脏病（肺心病）心力衰竭（心衰）的临床疗效。方法：选取肺心病急性发作期并心衰患者７２例，随机分成２组。对照组３６例采用常规综合治疗；治疗组３６例在对照组治疗基础上加用酚妥拉明和刺五加注射液静滴，每日１次，１０～１５日为１个疗程。２组均治疗１个疗程后评定疗效。结果：治疗组显效２３例，有效１１例，无效２例，总有效率９４．４％；对照组显效１１例，有效１５例，无效１０例，总有效率７２．２％，治疗组疗效明显优于对照组（χ２＝４．９０，Ｐ＜０．０５）。治疗组治疗后各种心律失常的发生率明显减少（Ｐ均＜０．０１），血液流变学各项指标明显改善（Ｐ均＜０．０１）；而对照组治疗前后变化不明显。结论：肺动脉高压和高血粘度是肺心病最重要的病理基础，加用酚妥拉明和刺五加注射液治疗肺心病心衰疗效显著，无明显不良反应。
Objective:To evaluate the clinically curative efficacy on heart failure of cor pulmonale using phentolamine combining Ciwujia Injection Methods:72 cases with acute seizure period of cor pulmonale were selected and divided into two groups randomly.The conventionally general therapy was given to control group ( n =36),meanwhile based on the therapy of control group phentolamine and Ciwujia Injection infused intravenously once a day were added in treatment group ( n =36).The therapeutic course was 10 15 days .After one therapeutic course the curative efficacy was evaluated.Results:There were markedly effective cases ( n =23),effective ( n =11),noneffective ( n =2),and total effective rate (94 4%) in treatment group but 11,15,10,and 72 2% respectively in control group.The curative efficacy in treatment group was significantly superior to that in control group (χ 2=4 90, P <0 05).After therapy the morbidity of assorted arrhythmias in treatment group was significantly reduced (all P <0 01).All the parameters of blood rheology were significant improvements (all P <0 01)in treatment group otherwise there were not obvious variations in control group before and after therapy.Conclusions:The hypertension of pulmonary artery and the increase in blood viscocity are the most important pathologic basis in cor pulmonale.Adding phentolamine and Ciwujia Injection.the curative effects on heart failure of cor pulmonale are significant and there are no sideeffects.
