高渗盐羟乙基淀粉对心内直视手术患者凝血功能的影响

Effect of Hypertonic Sodium Chloride Hydroxyethyl Starch Injection in Operation of Cardiopulmonary Bypass

摘要 目的通过高渗氯化钠羟乙基淀粉40注射液(HSS40)用于体外循环预充液中,观察对患者凝血功能、肾功能的影响。方法选择40例行体外循环心内直视术换瓣患者,根据人工心肺机预充液配方随机分为两组:Ⅰ组:为6%羟乙基淀粉130/0.4(万汶)1000m l和乳酸林格氏液;Ⅱ组:6%羟乙基淀粉130/0.41000m l和高渗氯化钠羟乙基淀粉40注射液适量4m l/kg,用乳酸林格氏液填充至2000m l。在体外循环前(T1),术毕(T2)监测:PT、FIB、APTT、BUN、Cr。结果①PT、APTT、FIB两组组内比较均有差异(P<0.05);②BUN两组组内比较均有差异(P<0.05),Cr两组间组内比较均无明显差异(P>0.05)。结论适量预充HSS40(4m l/kg)对换瓣患者的凝血,肾功能无明显影响,可安全应用于临床。 Objective To evaluate the influence of hypertonic sodium chloride hydroxyethyl starch 40 injection(HSS40)for priming solution on blood coagulation function and renal function of patients during cardiopulmonary bypass.Methods Forty patients with scheduled valve(s) replacement(s) were randomly divided into two groups;groupⅠ: 6% hydroxyethyl starch 130/0.4(voluven) 1000ml and lactate Ringer's solution;Ⅱ ∶ 6% hydroxyethyl starch 130/0.4 1000ml and hypertonic sodium chloride hydroxyethyl starch 40 injection amount 4ml/kg,with the final lactate Ringer's solution to fill 2000ml.In Before CPB(T1),at the end of surgery(T2): coagulation: PT,FIB,APTT;InT1,T2: renal function: BUN,Cr.Results ①Within two groups on PT,APTT,FIB,there were significant differences compared with these of T1 in groupⅠand Ⅱ(P0.05).②Within two groups on BUN,there were significant differences compared with these of T1 in groupⅠand Ⅱ(P0.05).While compared between two groups on Cr,there were no significant difference with groups and time points(P0.05).Conclusion HSS40(4ml/kg) has not influence on blood coagulation and renal functions.It may be valuable for clinical application.
