摘要
目的总结非梗阻性胆源性胰腺炎早期腹腔镜手术治疗的效果及护理体会。方法选择45例非梗阻性胆源性胰腺炎患者资料,先予保守综合治疗,病情缓解后予腹腔镜下胆道手术治疗,根据住院期间是否行胆道手术分为手术组和保守组,了解其治疗效果,电话随访,了解有无复发。结果 45例患者,手术组32例早期行腹腔镜胆囊切除术,5例行总胆管切开取石+T管引流。未见明显手术并发症,无手术死亡病例。随访手术组未见复发病例,保守组有2例胰腺炎复发。结论非梗阻性胆源性胰腺炎应注重心理护理,严密观察病情变化。早期手术治疗是安全的,可减少复发。
【Objective】To summarize the laparoscopic operation effect and nursing on non-obstructive acute biliary pancreatitis.【Methods】A total of 45 cases with non-obstructive acute biliary pancreatitis treated with conservative comprehensive treatment firstly,the operation group include patients who treated with laparoscopic biliary tract operation in duration of hospital stay,the conservative treatment group include patients who did not operate on biliary tract.Telephone follow-up the recurrence patients.【Results】There were 32 cases treated with laparoscopic cholecystectomy,in those cases,5 cases were treated with common bile duct stones fetching and T tube drain,all this group had no operation complications and death case,the follow-up showed no recurrence patients in operation group,and 2 recurrence patients in conservative treatment group.【Conclusion】For patient with non-obstructive acute biliary pancreatitis,it is worth to pay attention to mental nursing,and to closely observate on changes in condition.After remission,early laparoscopic operation is safe and can decrease the recurrence.
