摘要
目的探讨经脐入路行内镜微创保胆取石术的可行性。方法 8例胆囊结石患者行经脐入路腹腔镜保胆取石术,取脐部A、B、C点3个5mm切口,在腹腔镜直视下电凝切开胆囊底,用胆道镜取出结石,3-0可吸收线连续缝合胆囊。结果 8例手术均获成功。其中第1、2例手术时间分别为180及120min,最短为65min,平均114min。未放置引流管,无出血及胆漏,1例患者术后3d右侧腹疼痛,经消炎后缓解。平均术后5d出院。术后2周复查,脐部无明显手术瘢痕,B超胆囊无结石,胆汁透声好。结论经脐入路内镜微创保胆取石术是安全可行的,虽操作难度较常规腹腔镜保胆取石术稍大,但切口隐蔽,愈后疤痕不明显,美容效果好。
【Objective】To discuss the feasibility,and clinical application of transumbilical preservation of gallbladder and removal of calculus by laparoscopy.【Methods】Eights patients with gallbladder stones undergoing transumbilical endoscopic preservation of gallbladder and removal of calculus by laparoscopy.5 mm incisions revolve around unbilical region A,B and C points,incision the bottom of gallbladder with cautery hook in laparoscopic,take out stones with choledochoscope,continuous suture the gallbladder with 3-0 absorb stitch.【Results】All the operations were successfully performed.The operation time of the first case,the second and following cases about 180,120 and 90 min,average 114 min.No drainage tube was placed,postoperative bleeding and biliary leakage didn’t occur.The mean time of after operation hospitalization was 5 days.There was no visible scar on the abdominal wall.【Conclusions】Transumbilicals preservation of gallbladder and removal of calculus by laparoscopy is a safe and feasible operation.It is more difficult than laparoscopy cholecystectomy,cosmetic effect is wonderful and scar is invisible.
