摘要
目的:探讨超声引导内镜在诊治梗阻性黄疸的应用价值。方法:对120例梗阻性黄疸患者行超声引导内镜逆行性胆管造影(ERCP)术,内镜下鼻胆引流管放置(ENBD)术120例,内镜下乳头括约肌切开取石(EST)术70例,经内镜放置可膨式金属胆道支架(EMBE)术30例,并行X线内镜逆行性胆管造影(ERCP)术对照。结果:①本组超声引导ENBD术120例,全部放置成功,行X线对照,120例安放鼻胆引流管符合率为100%。②超声引导行EST术70例,取石成功率95.7%(67/70),术后超声提示结石完全取出57例,结石残留5例,经X线ERCP对照,超声总符合率92.5%(62/67),并发症发生率8.6%(6/70)。③超声引导经内镜EMBE术成功率93.3%(28/30)。结论:超声引导内镜诊治梗阻性黄疸,简便快捷,安全有效,可部分替代X线引导,具有经济、无辐射、无过敏等优点,有较高的临床应用价值。
Objective：To investigate the clinical value of ultrasound-guided endoscopic diagnosis and therapy for obstructive jaundice. Methods：We used ultrasound-guided endoscopic retrograde cholangiopancreatography（ERCP） for 120 obstructive jaundice patients, endoscopic nasal biliary drainage（ENBD） for 120 patients, endoscopic sphincterotomy（EST） for 70 patients, and endoscopic metal biliary endoprothesis （EMBE） for 30 patients,then we compared those with the operation of X-ray ERCP. Results： O120 cases of ENBD operations were all successfully completed. After the contrast to the X ray, coincidence ratio reached 100%. （2） Under the ultrasonography-guided, 70 cases of EST operations, success ratio of taking reached 95.7 % （67/70）. After the operation, ultrasonography told 57 cases of completely taken-out calculus with 5 negative cases. After the X-ray contrasting to ERCP coincidence ratio of the ultrasonography reached 92.5%（62/67）, the ratio of syndrome reached 8.6% （6/70）. （3） Under the ultrasonography-guided, ratio of succeeding complete EMBE reached 93.3% （28/30）. Conclusion： Therapy and diagnosis of obstructive jaundice with the ultrasound-guided endoscopic are easy,swift, safe, effective, and can replace the guide of X-ray partly, and are economic, unradiant, unallergic, owning a great clinic application value.
