急性脑梗死患者血清血管内皮生长因子和基质金属蛋白酶-9水平的动态变化及其意义 被引量:10

Dynamic Changes of Serum VEGF and MMP-9 in Patients with Acute Cerebral Infarction and Its Clinical Significance

摘要 目的探讨急性脑梗死患者血清血管内皮生长因子(VEGF)和基质金属蛋白酶-9(MMP-9)水平的动态变化及其临床意义。方法选取2004年6月—2009年5月我院神经内科收治的首次发病48 h内的急性脑梗死患者108例为脑梗死组,60例健康体检者为对照组。在急性脑梗死患者发病后1、3、5、7、14 d时测定其血清VEGF和MMP-9水平,并且对患者进行梗死面积测定和预后评估,分析VEGF和MMP-9水平与梗死面积及患者预后的关系。结果脑梗死组与对照组相比,各时点的血清VEGF和MMP-9水平显著增高(P<0.01),且呈动态变化。脑梗死组各时点VEGF和MMP-9的水平间无相关性(P>0.05)。血清VEGF和MMP-9水平与梗死面积均有一定相关性。预后不良组患者血清VEGF和MMP-9水平在发病后各时间点均明显高于预后良好组(P<0.01)。结论急性脑梗死患者血清VEGF和MMP-9水平均明显增高,且患者梗死面积和预后情况与VEGF和MMP-9的水平有密切关系。 Objective To investigate the dynamic changes and clinical significance of serum vascular endothelial growth factor(VEGF)and matrix metalloproteinase-9(MMP-9) in patients with acute cerebral infarction(ACI).Methods One hundred eight ACI patients admitted to our hospital from June 2004 to May 2005 were chosen as ACI groups,60 healthy subjects as control.In ACI group,serum VEGF and MMP-9 levels were determined on days 1,3,5,7,14 after onsets,and the infarct size measured and prognosis evaluated,the relationship of VEGF and MMP-9 to infarct size and prognosis were analyzed.Results Serum VEGF and MMP-9 levels at varying time points increased significantly in ACI group as compared with control(P0.01),showing dynamic changes.There was no correlation in VEGF and MMP-9 levels between different time points in group ACI(P0.05).Serum VEGF and MMP-9 were correlated with infarct size.VEGF and MMP-9 were significantly higher in patients with poor prognosis than in those with good prognosis at varying time points(P0.01).Conclusion Serum VEGF and MMP-9 levels increase significantly in ACI patients,and infarct size and prognosis are closely related to the levels of VEGF and MMP-9.
