摘要
目的探讨急性缺血性脑血管病患者血浆溶血磷脂酸含量(LPA)变化及蕲蛇酶对其影响。方法急性缺血性脑血管病患者100例,根据脑血管病分类及临床诊断标准分为急性脑梗死组ACI、短暂性脑缺血发作组TIA、椎-基底动脉供血不足组;急性脑梗死组根据神经功能缺损程度分为轻型组、中型组、重型组;根据疾病预后分为死亡组、生存组;部分急性脑梗死患者根据治疗方法分为蕲蛇酶组及常规治疗组。50例健康者为对照组。分别测定发病不同时间血浆溶血磷脂酸含量。结果 ACI组血浆LPA值较对照组明显增高,比较有显著性差异;TIA组、ACI组、椎-基底动脉供血不足组3组间比较有显著性差异,椎-基底动脉供血不足组与对照组比较无显著性差异;轻度、中度、重度功能障碍组血浆LPA含量的比较均有显著性差异,且ACI患者LPA含量与其临床神经功能缺损程度评分呈正相关;蕲蛇酶组治疗后3天、7天1、0天血浆LPA含量均明显低于常规治疗组,治疗后10天与正常对照组比较差异无显著性;常规治疗组血浆LPA含量在治疗后14天与正常对照组差异无显著性。结论①短暂性脑缺血发作和急性脑梗死患者血浆LPA含量均显著升高,表明此时血小板有可能处于活化状态,预警血栓形成的危险,有助于急性特别是超早期脑梗死诊断。②急性脑梗死患者血浆LPA含量越高,神经功能缺损程度越大,预后越差。③蕲蛇酶具有抗血小板聚集作用,尽早应用,有助于对急性脑梗死患者的神经功能恢复。
Objective Bjective Explore the acute ischemic cerebmvascular disease plasma were lysophospholipids acid level changes and e. snake enzyme on the influence, in order to guide the acute ischemic cerebrovascular disease, early diagnosis and prognosis assessment and drug treatment, etc. Methods In acute ischemic cerebrovascular disease 100 cases, according to the clinical diagnosis and cerebrovascular disease classification standard into acute cerebral infarction, transient ischemic attack group,vertebrobasilar artery blood supply shortage. Acute cerebral infarction groups according to the degree of neurologic deficits into light, medium, heavy group. According to the disease prognosis is divided into death, living groups. Parts according to the treatment of acute cerebral infarction were divided into conventional treatment group and Acutobin treatment group. And in 50 cases for healthy controls. Determination within the time required, plasma were lysophospholipids acid level. Results Acute cerebral infarction group than controls plasma LPA significantly higher, more significantly. Transient ischemic attack group, the noise of acute cerebral infarction, vertebrobasilar artery blood supply shortage group 3 groups have significant difference, vertebrobasilar artery blood supply shortage and control groups non - significance difference. Mild, moderate, and severe dysfunction group than in the plasma LPA levels significantly, and patients with clinical fie LPA level neurologic deficits score positively correlated, after the treatment group on Acutobin 3d, 7 d, 10 d plasma LPA levels were significantly lower than conventional treatment group. Conclusion treatment groups in treatment of plasma LPA level after 14 d and normal control group had no significant differences. Conclusion 1Transient ischemie attack patients with acute cerebral infarction were significantly eleated plasma LPA level, there may be in this platelet activation, help acute cerebral infarction, espaeially super early diagnosis. 2 Patients with acute cerebral infarction plasma LPA leel, neurologic deficits degree, the worse prognosis. 3 Acutobin with platelet aggregation, as soon as possible, it is helpful to application of patients with acute cerebral infarction neurological reeovery.
