摘要
本组胃癌术后再手术６８例，占同期胃癌病例的６７６％，其中复发癌，种植及转移癌５２例，局部复发９例，肝转移癌１１例、切口种植癌１３例、卵巢转移癌５例、再发癌２例、腹腔感染５例，十二指肠残端及吻合口瘘、吻合口出血各３例。结果，复发癌中９例为局部复发，内７例再除，切除率为７７％，肝转移癌１１例，其中３例姑息性切除，余８例注入无水酒精，加肝动脉插管化疗。５例卵巢转移癌均手术切除，１３例切口种植癌中１２例手术切除，切除率为９２３％，再手术死亡８例。结论：首次手术应按肿瘤观念和无菌术观念进行规范化操作。
To analyse the causes of reoperation of post gastric carcinoma operation and to find the methods of prevention.1004 Cases of gastric carcinoma injected to surgica operation during 1985～1996 were analysed,68 of cases underment reoperation(6 76%).The cause of reopertion were as follows recurrent carcinema and carcinoma metastaticum,52 cases,Among these patients,part-recurrent carcinoma,11 cases carcinoma metastaticum of hepar,implant carcinoma of intision 13 cases,carcinoma metastomotic of ovary 5 cases,and abdominal infection 5 cases;anastomotic hemerrhage 3 cases,leakage of duodenal stump and leakage of anastomosis 3 cases.follow up showed that 7 patients of recurrent carcinoma were reoperated(77%).Among 11 cases of carcinomas metastaticum of hepar,3 patients pallitative resection,8 patients percutaueous intratumoral injection of ethanal of 99%,combines with introarterial chemetherapy,5 patients of carcinoma metasticum of ovary were all reoperated (100%),12 patients of implant carcinoma of incision were reoperated (92 3%),8 patients died after operation.The fist operations should be performed strictly according to the standard non tumor operation rules to reduce the operation complication of post gastric carcinoma operation.
