COPD病人气道的多层螺旋CT低剂量电影扫描 被引量:22

摘要 目的探讨多层螺旋CT电影扫描对显示COPD病人中央和外周气道在呼吸周期中改变的价值。方法选择COPD病人67例,其中COPD0级12例,1级16例,2级16例,3级14例,4级9例;对照组15例。采用低剂量MSCT在右肺上叶尖段支气管水平行同层电影扫描。由两位高年资影像诊断医师在肺窗下(窗宽1000HU,窗位-500HU)以电影方式观察外周气道和中央气道在呼吸周期中的开闭情况。结果随着COPD病情的加重,外周气道和中央气道出现呼气期间狭窄明显增加,外周气道出现早于中央气道,但外周气道各级之间无统计学差异。结论 MSCT低剂量电影扫描可以作为研究和观察气道弹性的客观、敏感的方法。 Objective To study dynamic airway change in COPD during respiration. Methods Sixty seven COPD patients were selected, including 12 stage 0 patients,16 stage1 petients,16 stage 2 patients, 14 stage3 patients, 9 stage4 patients. The upper lobes , which contained the apical bronchus of right upper lobe, were examined by lowdose MSCT cine scan during respiration. The dynamic changes of central and peripheral airways were evaluated by two radiologists using cine method. Results With the aggravation of COPD, peripheral and central airways occurred apparently narrow during expiration. It happened earlier in peripheral airway than in central airway. Conclusion Low-dose MSCT cine scan is an objective, sensitive method for observing central and peripheral airway of COPD.
