摘要
目的:初步探讨DWI的量化指标表观扩散系数(ADC)值是否有助于鉴别ⅠA期子宫内膜癌、正常子宫内膜及变性的黏膜下肌瘤。方法:对经手术病理证实的ⅠA期子宫内膜癌28例、正常子宫内膜30例和变性的黏膜下肌瘤18例行常规MRI序列和轴位DWI检查(b=1000s/mm2),获取DWI图像并经工作站后处理获得ADC图,分别测量并比较ⅠA期子宫内膜癌病灶、正常子宫内膜及变性的黏膜下肌瘤病灶的ADC值。结果:ⅠA期子宫内膜癌、正常子宫内膜及变性的黏膜下肌瘤在T1WI上呈等信号,在T2WI及DWI上均呈高信号,肉眼不能鉴别三者;而三者的ADC值(10-3mm2/s)分别为(0.878±0.185)、(1.346±0.246)和(1.334±0.399),ⅠA期子宫内膜癌病灶的ADC值与后两者两两比较均有统计学差异(P均<0.05)。结论:ADC值的测量有助于鉴别ⅠA期子宫内膜癌与正常子宫内膜和变性的黏膜下肌瘤,DWI与常规MRI序列相结合能为内膜癌的鉴别诊断提供更多的辅助信息,是常规MRI的有益补充。
Objective: To initially investigate whether the apparent diffusion coefficient (ADC) values of uterine endometrial carcinoma staged I A differs from those of normal endometrium and degenerative submueosal leiomyomas. Methods: Studied population included 28 consecutive females with surgically proven uterine endometrial carcinoma staged IA, 30 females with pathologically confirmed normal en- dometrium in cervical cancer staged I patients and 18 females with surgically pathologically degenerative submucosal leiomyomas. All patients performed conventional MRI series and axial diffusion-weighted imaging (DWI) with b value of 1 000 s/mm2. ADC maps were obtained by post-processing from workstations. The ADC values of endometrial carcinoma staged I A, normal endometrium and degenerative submucosal leiomyomas were measured on the ADC maps and compared for the three groups. Results: All endometrial carcinoma staged I A, normal endometrium and degenerative submucosal leiomyomas appeared iso-intense on T1-weighed imaging, hyperintense on T2-weighed imaging and DWI. The ADC values (10-3 mm2/s) of endometrial cancer staged I A were (0.87 8±0.185), which were significantly lower (P〈0.05) than those of normal endometrium (1.346 ±0.246) and degenerative submucosal leiomyomas (1.334±0.399). Conclusion: The measurement of ADC values is help for differentiating endometrial carcinoma staged I A from normal endometrium and degenerative submucosal leiomyomas. The combination of DWI and conventional MR series can provide more potential information for differential diagnosis of endometrial carcinoma and DWI is helpful complement for conventional MR examination.
