摘要
目的:探讨有早产征象孕妇尿液基质金属蛋白酶-9(MMP-9)水平,预测早产的价值。方法:用双抗体夹心ELISA法,检测无早产风险和征象的妊娠32周妇女(A组),先兆早产或早产胎膜早破妇女,追踪其中72h内未分娩者(B组),72h内分娩者(C组)及足月临产者(D组)尿液MMP-9水平。结果:A,B,C,D组孕妇尿液MMP-9浓度分别为2.82±0.51,6.17±0.97,7.10±0.72,7.41±0.86ng/ml,A组显著低于其他各组,差异有显著的统计学意义(P<0.01),B组与C组间差异也有显著的统计学意义(P<0.01)。尿液MMP-9水平预测未来72h内分娩的ROC曲线下面积为0.89。结论:尿液MMP-9水平在临产、破膜后明显增高,用于预测早产结局有一定的价值。
Objective:To explore the value of urinary matrix-metalloproteinase-9(MMP-9) concentration in predicting labor by examining the urine of pregnant women with the sign of premature delivery.Method:The MMP-9 concentration was tested by using the ELISA technique in the target groups:pregnancy of 32 weeks without sign of premature delivery and other risks(Group A);pregnancy with threatened premature delivery or preterm premature rupture of the membrane(PPROM) who undelivered in 72 hours(group B),and who delivered in 72 hours(group C),labored in term pregnancy(group D).Result:The concentration of MMP-9 in each group was 2.82±0.51ng/ml,6.17±0.97ng/ml,7.10±0.72ng/ml,7.41±0.86ng/ml,group A was significantly lower than that of each other groups(P0.01),and there was a significant difference between group B and group C(P0.01).The area under ROC curve of MMP-9 concentration was 0.89 that used as an index of the delivery status in 72 hours.Conclusion:The urinary matrix-metalloproteinase-9(MMP-9) concentration rises obviously in the procedures of labor and rupture of membrane,which is worth to be further developed in premature delivery.
