年轻下腰痛患者椎间盘退变进展的临床研究 被引量:2

摘要 目的观察年轻的下腰痛患者,经10年随访之后再次行腰椎MRI检查,看其是否发生预期的严重下腰痛、腰椎间盘突出及腰椎功能障碍。方法 24例曾因下腰痛于1999年到我院就诊的患者,经腰椎MRI检查可见部分患者均存在不同程度的腰椎间盘退变,随访10后,于2009年再次对随访病例行腰椎MRI检查,根据MRI检查结果记录腰椎间盘退变及突出的状况。结果 1999年24例下腰痛患者中,经MRI检查证实16例患者存在不同程度的椎间盘退变,占所有病例的67%,共有18个椎间盘发生退变,与2009年再次对随访病例行腰椎MRI检查,结果所示能完成随访的22例患者中均存在至少一个呈不同程度退变的椎间盘,并且在随访开始时就已发生退变的18个腰椎间盘中,有15个在随访期间发生了椎间盘突出,突出发生率为83%,而随访开始时信号正常的椎间盘中,其椎间盘突出的发生率仅为20%,且在随访期间有2例患者因严重的腰椎间盘突出而行腰椎手术。结论年轻的下腰痛患者随时间推移,成年后其腰椎间盘退变程度将逐渐加重,且腰椎间盘突出症的发生几率也将大大增加,然而其下腰痛的严重程度及行腰椎手术的几率随时间推移却未见明显增加。
