运用综合策略实现缺血心肌再血管化 被引量:2

Ischemic myocardium revascularization through comprehensive strategy

摘要 目的:探讨外科综合策略对冠心病再血管化的价值。方法:回顾性分析900例冠状动脉旁路移植术患者的临床资料。结果:脱泵搭桥450例(转为体外循环下搭桥17例),浅低温体外循环心脏停跳下搭桥417例,常温体外循环辅助下心脏不停跳搭桥33例。共移植血管2579支(平均2.9支/例),其中左乳内动脉893支,游离右乳内动脉118支,左桡动脉232支,右桡动脉82支,大隐静脉1254支;全动脉化130例。863例治愈出院,住院期间死亡37例。结论:综合运用多种策略能够完成缺血心肌完全再血管化并取得良好效果。 Objective To discuss the value of comprehensive surgical strategy to coronary revascularization.Methods The clinical data of 900 cases receiving coronary artery bypass grafting were retrospectively analyzed.Results In 900 cases,450 patients received off-pump coronary artery bypass grafting,in which 17 cases converted to on-pump coronary artery bypass grafting,and the other 450 cases underwent on-pump coronary artery bypass grafting,in which 33 cases received on-pump beating-heart coronary artery bypass and 417 cases received conventional coronary artery bypass grafting.A total of 2 579 grafts were constructed with 893 left interial mammary arteries,118 free right mammary arteries,232 left radial arteries,82 right radial arteries,and 1 254 saphenous veins.Total arterial bypass grafting was feasible in 130 cases.Thirty-seven cases died in hospital and 863 cases were all cured.Conclusion A comprehensive use of surgical strategy can fulfill completely revascularization of myocardial ischemia and obtain good results.
