准分子激光原位角膜磨镶术切削光区直径对球差的影响 被引量:1

摘要 目的:探讨准分子激光原位角膜磨镶术(LASIK)不同直径治疗光学区术后球差的变化。方法:对157例(314眼)近视和近视散光眼分为低度、中度和高度近视,各类人群根据治疗光区直径5.0、5.5和6.0mm再分为a、b和c亚组进行LASIK,用博士伦Zywave波前像差仪测量术前和术后3个月6mm瞳孔直径下球差。结果:①术前低度近视组a、b和c亚组球差均方根值(RMS)分别为(0.12±0.08),(0.11±0.07)和(0.10±0.06);中度近视组各亚组球差RMS值分别为(0.11±0.07),(0.12±0.06)和(0.12±0.07);高度近视组各亚组球差RMS值分别为(0.13±0.08),(0.12±0.08)和(0.11±0.09)。术前低度、中度和高度近视组各亚组球差RMS值比较差异无统计学意义(F=0.565,0.624和0.376,P均>0.05)。②术后低度近视组a、b和c亚组球差RMS值分别为(0.34±0.06),(0.32±0.09)和(0.29±0.08);中度近视组各亚组球差RMS值分别为(0.41±0.09),(0.39±0.08)和(0.34±0.08);高度近视组各亚组球差RMS值分别为(0.49±0.06),(0.46±0.07)和(0.42±0.07)。术后低度、中度和高度近视组各亚组球差RMS比较差异有统计学意义(F=3.342,9.468和17.948,P均<0.05)。结论:LASIK术后球差随着手术切削区直径增加而减少,提示在保证角膜手术安全进行的范围内,可适当增加切削光区直径来减少球差,提高视觉质量。 Aim:To investigate different optic zone ablation diameter of spherical aberration changes after laser in situ keratomileusis(LASIK).Methods:157 cases (314 eyes) with myopia and myopic astigmatism eyes were divided into low,moderate and high myopia groups,In each group,eyes were divided into a,b and c subgroups according to optic zone ablation diameter(5.0 mm,5.5 mm,6.0 mm)for LASIK. Analyze the spherical aberration differences at the 6.0 mm pupil in each subgroup preoperation and 3 month postoperative with the Zywave aberrometer.Results:①The RMS of spherical aberration in preoperative of low myopia group a,b and c was (0.12±0.08),(0.11±0.07),and (0.10±0.06); moderate myopia group was (0.11±0.07),(0.12±0.06),and(0.12±0.07); high myopia group was (0.13±0.08),(0.12±0.08),and (0.11±0.09).The RMS of spherical aberration show no statistical significance of low,moderate and high myopia group a,b and c sub-group in preoperative(F=0.565,0.624,and 0.376,P0.05). ②The RMS of spherical aberration in postoperative of low myopia group a,b and c was (0.34±0.06),(0.32±0.09),and (0.29±0.08); moderate myopia group was (0.41±0.09),(0.39±0.08),and (0.34±0.08); high myopia group was (0.49±0.06),(0.46±0.07),and (0.42±0.07).The RMS of spherical aberration of low,moderate and high myopia group a,b and c sub-group were statistically significant(F=3.342,9.468,and 17.948,P0.05). Conclusion:Spherical aberration after LASIK reduced with increased the diameter of the ablation zone diameter,suggesting that in ensuring the safety of corneal laser refractive surgery,may be appropriate to increase the ablation zone diameter and reduce spherical aberration and improve visual quality.
