摘要
目的探讨2型糖尿病(T2DM)患者的血浆皮质醇(F)水平的变化及其与代谢指标的相关性。方法比较76例T2DM组和23例正常对照组的血浆8:00及4:00F、空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、体重指数(BMI)、腰臀比(WHR)、甘油三酯(TG)、总胆固醇(TC)等指标,并分析T2DM患者8:00F与其他代谢指标之间的相关性。结果 T2DM患者8:00及4:00F、FPG、2hPG、HbA1c、WHR、TG、TC均较正常对照组明显升高,差异有统计学意义(P<0.05);76例T2DM患者中有34例患者(48.7%)出现皮质醇分泌昼夜节律紊乱,表现为4:00皮质醇超过同一天8:00皮质醇的50%,与正常对照组相比P=0.002,有明显统计学意义;T2DM患者8:00F与FPG、HbA1c、BMI、WHR、TG、TC呈正相关,相关系数分别为0.548、0.430、0.940、0.796、0.669、0.612。结论 T2DM患者体内存在以血浆F(8:00及4:00)增高及F昼夜节律紊乱为主的HPA轴功能紊乱。
Objective To investigate type 2 diabetes mellitus（T2DM）patients cortisol levels and related to the correlation between the metabolic indices.Methods Compared with cortisol（at 8a.m.and 4p.m.）（F）,fasting plasma glucose（FPG）,2-h plasma glucose（2HPG）,hemoglobin A1c（HbA1c）,body mass index（BMI）,waist-hip ratio（WHR）,triglycerides（TG）,triglycerides（TC）between with T2DM and the normal control group,and analysis T2DM patients correlation between F at 8a.m.and the metabolic indices.Results T2DM patients F at 8a.m.and 4p.m.were significantly higher normal control group,the difference was statistically significant（P 0.05）.76 cases of type 2 diabetes in 34 patients appear cortisol secrete circadian rhythms disorder,for 4.p.m.cortisol results more than 50% of at 8 a.m.,compared with the normal control group,P = 0.002,has obvious statistical significance;T2DM patients F at 8a.m.with FPG,HbA1c,BMI,WHR,TG and TC had positively correlation,the correlation coefficients were 0.548,0.430,0.940,0.796,0.669 and 0.612.Conclusion T2DM patients have HPA axis functional disorder,for plasma F（morning at 8：00 and 4 p.m.）increased and F circadian rhythms functional disorder.
