摘要
目的:探讨痔上黏膜环切钉合术(PPH)配合芒痔汤熏洗坐浴治疗重度痔病的临床疗效和安全性。方法:对60例重度痔病患者行PPH术配合术后芒痔汤熏洗坐浴治疗,比较患者术前,术后1、6、30天的疼痛、水肿、坠胀和出血情况。结果:60例患者的平均手术时间为20.5min;平均住院天数为4.5天;术后疼痛、水肿、坠胀及出血情况较前明显缓解(P<0.05);术后并发症发生率低,以尿潴留(5%)和出血(1.7%)多见,无肛门失禁、肛周感染、吻合口狭窄者及直肠阴道瘘的发生。随访了58例患者,随访率为96.7%,术后随访2～18个月,无复发和其他不良现象。结论:PPH术具有手术时间短、创伤小,配合芒痔汤熏洗坐浴治疗重度痔病恢复快、并发症少等,患者满意度好,是治疗重度痔病的理想方法。
Objective：To investigate the clinical effect and security of PPH with hip bath using Mangzhi Decoction in the treatment of severe hemorrhoids.Methods ：60 cases with severe hemorrhoids were treated with PPH and hip bath using Mangzhi Decoction.The pain,edema,prolapse and bleeding in annal,before the procedure 1,6,30 days after the procedure were analyzed.Results ：The average operation and hospitalization time were 20.57 minutes and 4.5 days.The pain,edema,prolapse and bleeding after the operation were relived obviously.And the incidence rate of postoperation complications was low.Retention of urine（5%）and bleeding（1.7%）were the main complications.No fecal incontinence,perianal infection,stenosis and rectovaginal fistula were occurred after operation.58 cases were follow-up（97.7%）.With follow-up 2 to 18 months,no hemorrhoidal recurrence and untoward effect occurred.Conclusion ：PPH＇s advantages are shorter time of operation and minimal invasive.Hip bath using Mangzhi Decoction after the operation can help patients recover quickly and it has less complications and high satisfaction degrees.PPH and hip bath using Mangzhi Decoction is an effective measure for the treatment of the severe hemorrhoids.
