喷他佐辛防治瑞芬太尼全麻后急性痛觉过敏的临床观察被引量：15

摘要目的观察喷他佐辛防治瑞芬太尼急性痛觉过敏的疗效。方法选择择期妇科腹腔镜手术60例(ASAⅠ～Ⅱ级),随机分为3组,各组均采取气管内插管全凭静脉麻醉,术中以静脉靶控输注丙泊酚,持续输注瑞芬太尼,间断给予维库溴铵维持麻醉。P组(n=20):手术结束前15～20min给予喷他佐辛30mg(用0.9%生理盐水稀释至5mL)肌肉注射;K组(n=20):氯胺酮1.5mg/kg(用0.9%生理盐水稀释至5mL)肌肉注射;C组(n=20,对照)给予生理盐水5mL肌肉注射。观察记录3组恢复自主呼吸的时间、呼之睁眼时间、气管导管拔除时间、定向力恢复时间、苏醒后VRS评分及需要追加芬太尼镇痛的人数、有无恶心及呕吐、呼吸抑制及术后躁动情况。结果 3组患者自主呼吸恢复时间、苏醒时呼之睁眼时间、气管导管拔除时间差异无显著性(P>0.05),定向力恢复时间,K组较P组与C组明显延长(P<0.05);苏醒后VRS评分C组明显高于P组及K组(P<0.05),且拔管后需要追加芬太尼的人数明显少于C组(P<0.05),术后躁动P组明显少于K组及C组(P<0.05)。结论喷他佐辛30mg肌肉注射可有效减轻瑞芬太尼全身麻醉患者术后急性痛觉过敏。
