摘要
目的 探讨血清基质金属蛋白酶-9浓度结合宫颈长度的测量在预测自发性早产中的价值.方法 用酶联免疫吸附测定法检测25例孕28~36+6周先兆早产孕妇和16例相近孕周正常孕妇血清中基质金属蛋白酶-9的浓度,并同时经阴道超声检测宫颈长度.结果 25例先兆早产孕妇中11例发生早产,14例7天内未发生早产.发生早产组的基质金属蛋白酶-9浓度均值为39.04±9.41ng/mL,显著高于保胎成功组(7天内未发生早产)的30.55±1.65ng/mL(t=3.330,P〈0.05),亦高于正常对照组的26.72±8.17ng/mL(t=3.622,P〈0.05).同时,早产组的宫颈长度均值为16.75±3.05mm,显著低于保胎成功组的24.31±3.57mm(t=-5.598,P〈0.05),亦低于正常对照组的31.31±1.99mm(t=-15.063,P〈0.05).结论 孕妇血清中基质金属蛋白酶-9的浓度水平结合宫颈长度的检测可作为早产发生的预测指标.
Objective To determine predictive value of detection of serum level of matrix metalloproteinase-9 ( MMP-9 ) combined with measurement of cervical length(CL) for premature delivery. Methods ELISA method was used to determine serum level of MMP-9 of 25 pregnant women with threatened premature delivery at 28 - 37 weeks of gestation and 16 normal pregnant women at similar gestational weeks. At the same time, CL of the pregnant women was measured by using transvaginal ultrasonography. Results Among 25 pregnant women with threatened premature delivery, 17 pregnant women developed eventually premature delivery and 14 pregnant women didn' t develop premature delivery within 7 days. The mean serum level of MMP-9 of the pregnant women in the premature delivery group was 39.04 ± 9.41ng/mL, which was significantly higher than that of the pregnant women who didn' t develop premature delivery within 7 days (30.55 ± 1.65ng/mL) (t = 3. 330, P 〈 0.05 ) and that of normal pregnant women (26.72 ± 8.17ng/mL) (t = 3. 622, P 〈 0.05 ). And mean CL of the pregnant women in the premature delivery group was 16.75 ± 3.05mm, which was significantly shorter than that of the pregnant women who didn' t develop premature delivery within 7 days ( 24.31 ±3.57mm) ( t = - 5. 598, P 〈 0.05 ) and that of normal pregnant women (31.31 ± 1.99 mm) (t = - 15. 063, P 〈 0. 05 ). Conclusion Detection of serum level of MMP-9 combined with measurement of CL can be used as a predicting index of spontaneous premature delivery.
