摘要
目的：对比二维超声心动图多巴酚丁胺负荷试验（2-DSE）与彩色心室壁动力分析（colorkinesis，CK）多巴酚丁胺负荷试验（CK-DSE）识别存活心肌的价值。方法：急性和陈旧性心肌梗死患者共41例，全部应用二维超声心动图和CK技术做了小剂量多巴酚丁胺负荷试验（LDSE）。结果均与其中15例经皮冠状动脉腔内成形术（PTCA）后2～6个月的超声心动图检查和29例做了硝酸甘油酯介入99m锝-甲氧基异丁基异腈单光子发射计算机断层摄影术（NG-ECT）的结果分析比较。结果：经皮冠状动脉腔内成形术后进行超声心动图检查的15例和接受NG－ECT检查29例中2D－DSE和CK－DSE各自检出存活及坏死节段与经皮冠状动脉腔内成形术后二维超声心动图检查的一致性分别为81.0％（34／42）和85.0％（34／40），P＞0．05；与NG－ECT的一致性分别为85．1％（63／74）和84.7％（61／72），均无显著性差异。结论：CK－DSE与2D－DSE相比识别存活心肌的结果同样可靠，还似更便捷、客观，可以应用于临床。
Objective: To assess the value of color kinests dobutamine stress echocardiography(CK--DSE)in identifying viable myocardium.Methods: Forty-one patients with myocardial infarction were studied. Low-dose dobutamine stress test (LDSE )was performed on all patients. Of which, 15 patients had underwent successful percutaneous transluminal coronary angioplasty(PTCA) and echocardiography 2-6 months post PTCA and 29 patients had performed myocardial nitroglycerin 99mTc-MIBISPECT(NG-ECT). A 16-segments model was used in echocardiography analysis.Results:Viable and infarcted segments were detected by 2D-DSE and CK-DSE respectively in 15 patients who underwent PTCA,and the consistency were 81.0 % (34/42) and 85.0% (34/40) respectively (P＞0. 05). In 29 patients who underwent NG-ECT, the consistency was 85. 1% (63/74 ) and 84. 7% (61/72 ) in 2D-DSE and CK-DSE respectively (p＞0. 05). There was no significant difference between two techniques.Conclusion:Compared with 2D-DSE and CK-DSE,the results of identifying viable myocardium are also a reliable andeasy technique in identifying viable myocardium.
