摘要
目的探讨厄贝沙坦联合阿魏酸钠注射液治疗早期糖尿病肾病的疗效。方法将60例早期糖尿病肾病患者随机分成对照组和治疗组,对照组在给予常规饮食、运动疗法和药物治疗基础上给予厄贝沙坦150mg/d口服,1次/d,治疗组在对照组基础上加用阿魏酸钠注射液0.3g+生理盐水100ml静脉滴注,1次/d,疗程均为4周。观察、记录治疗前后24h尿微量白蛋白(u-ALB)、空腹血糖(FBG)、血尿素氮(BUN)、肌酐(Scr)、胆固醇(TC)、三酰甘油(TG)、血液流变学指标、平均动脉压(MAP)和药物的不良反应。结果两组治疗后u-ALB、MAP、FBG较治疗前明显下降,差异有统计学意义(P<0.05),治疗组治疗后u-ALB、TC、TG、纤维蛋白原、全血粘度、血浆粘度、血细胞比容、MAP下降较对照组明显,差异有统计学意义(P<0.05)。两组治疗后FBG、BUN、Scr比较,差异无统计学意义。结论厄贝沙坦联合阿魏酸钠注射液治疗早期糖尿病肾病,能显著降低尿微量白蛋白、MAP、降血脂、降低血液粘滞度、改善微循环、减轻肾脏损害、延缓糖尿病肾病的发展。两药联合使用未见明显不良反应增加。
Objective This study investigated the effects and mechanisms of the combined use of irbesartan and sodium ferulate injection for treatment of early diabetic nephropathy（DN）.Methods 60 cases of early DN patients were randomly divided into 2 groups.In the control group（n=30）,the patients were received irbesartan 150mg/d,while in the experiment group（n=30） received irbesartan 150mg/d,asoociated with sodium ferulate 0.3g added in 0.9% solution of sodium choride 100ml together driped by vein,qd.Treatment courses lasted 4 weeks in both groups.U-Alb、FBG、BUN、Scr、TG、TC and MAP were deteded for all patients before and after therapy,the data obtained from two groups were compared each other and analysed statistically.ResuitsDetected outcomes indicated that,after therapy,all patients,MAP,TC,TG,whole blood viscosity,viscosity of plasma,Fibrinogen and packed cell volume were all significantly reduced in the experiment group,it was more obvious than in the control group.Conclusion Combination of irbesartan and sodium ferulate injection with conventional western medication has a better effect on the treatment of early DN than only Irbesartan with conventional western medication.
