不同膳食纤维对重型颅脑损伤患者肠屏障的影响 被引量:3

摘要 目的探讨不同膳食纤维对严重颅脑损伤患者肠屏障保护作用。方法颅内损伤患者肠内营养给予能全素(不含膳食纤维)、能全力、复合膳食纤维,检测血浆中内毒素(1ipopolysaccharide,LPS)、二胺氧化酶(diamineoxidase,DAO)、D-乳酸含量,并评价在入院时,入院后14、28 d格拉斯哥评分(Glasgow Coma Scale,GCS)评分,进行比较分析。结果在入院或手术后1 d,能全素组、能全力组、复合膳食纤维组LPS、DAO、D-乳酸水平差异无统计学意义(P>0.05);能全力治疗组和复合膳食纤维组与能全素组相比,LPS、DAO、D-乳酸水平在7 d和14 d明显降低(P<0.05,P<0.01),其中复合膳食纤维组降低更明显。在入院或手术后1 d,能全素组、能全力组、复合膳食纤维组GCS评分差异无统计学意义(P>0.05),能全力治疗组和复合膳食纤维组与能全素组相比,GCS评分在7 d和14 d明显增高,其中复合膳食纤维组GCS评分增高更明显。结论在重型颅脑损伤患者中,给予含有膳食纤维的肠内营养对肠屏障具有明显的保护作用,但以给予复合膳食纤维保护作用最为明显。膳食纤维具有调节肠道功能,对患者的恢复具有明显的保护作用。
