摘要
目的对精神病专科医院耐药菌感染现状进行调查分析,探讨专科医院进行耐药菌感染预防和控制策略。方法回顾分析2005-2009年所有标本中耐药菌的分离率。结果分离出耐药菌株共60株,其中大肠埃希菌30株,占50.00%;MRSE 10株,占16.67%;MRSA 7株,占11.67%;人葡萄球菌5株,占8.33%;铜绿假单胞菌1株,占1.67%;其他7株,占11.67%;大肠埃希菌产ESBLs检出率86.67%,标本涉及痰、粪、小便及切口分泌物;其他均为耐甲氧西林菌株;革兰阳性耐药菌多发生在肺部感染和口腔感染病例中,而肠杆菌科耐药菌则多发生在泌尿道感染病例中。结论精神病专科医院耐药菌感染病例院内播散的危险确实存在,多药耐药菌出现的频率增加;进一步加强耐药菌的监测,提高病原菌及药敏试验送检率,合理应用抗菌药物,可防止耐药菌感染病例的院内播散。
OBJECTIVE To investigate the drug-resistant bacteria infections in the psychiatric hospital, to discuss the prevention and control strategies. METHODS The isolation rates of drug resistant bacteria from all specimens during 2005 -- 2009 were retrospective analyzed. RESULTS 60 drug resistant strains were isolated, including 30 strains of Escherichia coli AD, accounting for 50. 00% ; MRSE 10 strains, accounting for 16. 67% ; MRSA 7 strains, accounting for 11. 67%; 5 of Staphylococcus aureus, accounting for 8. 33% ; 1 strain of Pseudornonas aeruginosa, accounting for 1. 67%; other 7 strains, accounting for 11. 67%. The detection rate of E. coli producing ESBLs AD was 86. 67%, the specimens involved sputum, feces, urine and wound secretion; others were methicillin-resistant strains. Resistant Gram-positive drug resistant bacteria mainly occurred in the lungs and mouth infections, but Enterobacteriaceae mainly occurred in urinary tract infections. CONCLUSIONS The risk of psychiatric hospital spread of drug resistant bacteria nosocomial infection does exist, which increases the frequency of multiple drug resistant bacteria. To further strengthen the monitoring, improve the detection rate of pathogenic bacteria and drug sensitivity test and rationally apply the antibiotic can prevent the spread of drug-resistant bacteria nosocomial infections.
