摘要
目的:通过分析节细胞神经瘤的MSCT影像特征及病理表现,加深对该疾病的认识,以期提高对其诊断的准确性。方法:回顾性分析术后24例节细胞神经瘤的MSCT影像特点与病理表现。结果:24例神经节细胞瘤病例中,位于肾上腺7例、腹膜后11例、后纵隔3例、盆腔2例,盆腔及后纵隔同时发病者1例,共计26个病灶。26个病灶中7个呈嵌入性生长,包绕或推移周围血管,但血管形态正常。CT平扫19个病灶表现为均匀低密度,7个表现为混杂低密度,16个病灶内可见散在点状或小斑块状钙化;增强扫描后4个病灶未见强化,18个病灶可见条片状或斑片状不均匀轻中度进行性强化,4个病灶表现为团块状或斑片状不均匀明显强化,6个病灶可见包膜强化。术后组织病理示:肿瘤主要由成熟的神经轴突、神经束和纤维组织构成,其间散在成熟的节细胞、Schwann细胞。其中4个明显不均匀强化的病灶内部分区域见分化不成熟的节细胞。结论:节细胞神经瘤的MSCT影像特征与病理表现具有高度的一致性,正确认识其MSCT表现,可对绝大多数病例做出正确的诊断。
Objiective To analyze the correlation between multi-slice CT(MSCT) scans and histologic findings of ganglioneuroma and multi-slice CT(MSCT) features compared with histopathology and to improve the diagnostic ability performance of ganglioneuroma.Methods MSCT Imaging scans findings and histopathological features in 24 patients with ganglioneuroma were analysisanalyzed retrospectively.Results Among 24 patients with ganglioneuroma,7 patients with lesions located in the adrenal gland,11 cases with lesions located in the retroperitoneum,3 cases with lesions located in the posterior mediastinum,2 cases with lesions locate in the pelvis,1 cases with two lesions located in the posterior mediastinum and cavitas pelvis at the same time.Of total 26 lesions,7 lesions showed a tendency of wedging into the space encasing and lapsing the vessel.On plain scan,19 lesions showed homogeneous low density and 7 were inhomogeneous.16 lesions had scattered and patchy cafilication.The obvious heterogeneous enhancement was found in 4 cases,mild to moderate progressive enhancement was in 16 cases,no enhancement was in 4 cases.The capsules enhancement was demonstrated in 6 cases.Pathology showed that ganglioneuroma contained mature ganglion cells,Schwann cells,neurites and sustentacular fibers.The ganglioneuroma was partially composed of immature and atypical cells in 4 cases with obvious heterogeneous enhancement.Conclusion MSCT scans can show high agreement with histopathological findings of some imaging characteristics in ganglioneuroma,which can help make the diagnose preoperatively in most patients.
