摘要
目的比较卡马西平和奥卡西平单药治疗儿童额叶癫的疗效。方法收集本院新诊断的额叶癫47例儿童。男29例,女18例;年龄3～12岁;发作频率17～40次.d-1。根据就诊顺序按单双号顺序分成2组:卡马西平组和奥卡西平组,分别给予卡马西平和奥卡西平单药治疗。奥卡西平起始剂量8 mg.kg-1.d-1,每周增加5 mg.kg-1,逐渐加量至15～30 mg.kg-1.d-1,最大剂量40 mg.kg-1.d-1;卡马西平起始剂量5 mg.kg-1.d-1,每周增加5 mg.kg-1,逐渐增加剂量至10～20 mg.kg-1.d-1或其血药质量浓度达6～10 mg.L-1。随访6个月后评价2组疗效及不良反应。采用SPSS10.0软件进行统计学分析。结果共45例完成观察和随访,卡马西平组22例,奥卡西平组23例。卡马西平组13例完全控制发作(59.1%),3例部分控制(13.6%),其中3例随访6个月时EEG检查正常。奥卡西平组9例控制发作(39.1%),4例部分控制(17.4%),其中5例随访6个月时EEG正常。二组完全控制率、总有效率及EEG的改善比较均无统计学差异(Pa>0.05)。结论奥卡西平和卡马西平单药治疗儿童额叶癫均有很好疗效和耐受性。
Objective To compare the efficacy of carbamazepine and oxcarbazepine in children with frontal lobe epilepsy as monotherapy.Methods Forty-seven children（29 male,18 female;3-12 years old） with childhood frontal lobe epilepsy were enrolled and evaluated.They had 17-40 seizures once a day.Eligible patients were divided into 2 groups,and the patients were treated with carbamazepine in CBZ group or oxcarbazepine in OXC group.The initial dosage of OXC was 8 mg·kg^-1·d^-1nd gradually increased every week（5 mg·kg-1） and maintenance dosage was 15-30 mg·kg^-1·d^-1he maximum dosage was 40 mg·kg^-1·d^-1he initial dosage of CBZ was 5 mg·kg^-1·d^-1nd was weekly increased by 5 mg·kg-1 weekly until the dosage amounted to 10-20 mg·kg^-1·d^-1r a plasma concentration of 6-10 mg·L^-1.All patients had at least been followed up 6 months.SPSS 10.0 software was used to analyze the data.Results Forty-five children completed the study,23 children took OXC,and 22 children took CBZ.Thirteen children were seizure-free（59.1%）,3 cases were partially controlled（13.6%） in CBZ group,and 3 cases of them showed normal electroencephalography（EEG） at the end of the 6 months;In the OXC group,9 children were seizure-free（39.1%）,4 cases were partially controlled（17.4%）,and 5 cases of them showed normal EEG at the end of the 6 months.Severe adcerse effects were not found in all patients（Pa〉0.05）.Conclusions Both CBZ and OXC are effecive in treating typical frontal lobe seizures.
