摘要
目的探讨双胎新生儿发育及其罹患疾病与其双胎胎盘血管吻合的关系。方法以2008年9月-2009年8月在本院出生后转入NICU的14对(28例)双胎新生儿为研究对象,根据灌注后的双胎胎盘血管吻合情况,分为存在明显血管吻合组(A组)和未见明显吻合组(B组),调查2组患儿体质量、头围、身长及其所罹患的疾病情况,并对其进行新生儿神经行为测试(NBNA)、婴幼儿智力发育测试(CDCC)评分,比较其与胎盘血管吻合之间的相关性。结果 A组患儿体质量、头围、身长与B组比较有统计学差异(t=6.070、5.237、5.784,Pa<0.01),NBNA、CDCC评分(t=-3.409、5.262,Pa<0.01)及新生儿窒息、新生儿高胆红素血症、HIE、新生儿呼吸窘迫综合征患病率与B组比较亦有统计学差异(t=7.529、7.964、5.535,Pa<0.05)。A组大新生儿与小新生儿体质量、头围、身长,NBNA评分、CDCC评分比较均存在统计学差异(t=-2.046、-4.400、-3.751、-4.983、-3.494,Pa<0.05);B组大新生儿与小新生儿之间在体质量、头围、身长,NBNA评分、CDCC评分比较无统计学差异。结论双胎新生儿各自的发育及其罹患疾病的情况与其双胎胎盘的血管是否存在明显吻合存在一定的关系。
Objective To investigate the relationship of placental vascular anastomosis and physical development and morbidity of the disease in twin neonates.Methods Fourteen pairs of twin neonates deliveried from Sep.2005 to Aug.2009 were enrolled in Newborn Intensive Care Unit,the Third Affiliated Hospital of Zhengzhou University.These twins were divided into 2 groups according the conditions of placental vascular anastomosis：significant placental vascular anastomosis group（group A） and no significant vascula anastomosis group（group B）.Birth weight,head circumference,length,the morbidity of disease were all investigated in 2 groups.Clinic follow-up included neonatal behavioral neurological assessment（NBNA） and children′s development center of China（CDCC）.The correlation of neonates placental vascular anastomosis between twin neonates were compared.Results There were statistically significant differences between group A and group B in birth weight,head circumference and body length（t=6.070,5.237,5.784,Pa0.01）,NBNA and CDCC scores（t=3.409,5.262,Pa0.01）,the morbidity of the diseases such as asphyxia,neonatal hyperbilirubinemia,neonatal hypoxic-ischemic encephalopathy,neonatal respiratory distress syndrom（t=7.529,7.964,5.535,Pa0.05）.There were statistically significant differences between the 2 neonates of co-twin in birth weight,head circumference,body length,NBNA and CDCC scores（t=-2.046,-4.400,-3.751,-4.983,-3.494,Pa0.05） in significant placental vascular anastomosis group,but no significant difference was found in group B.Conclusion There were association between the conditions of placental vascular anastomosis and twin neonates development and the morbidity of the diseases in twins.
