摘要
目的探讨深圳市宝安区2007年0～8月龄儿童麻疹高发的原因,以找出针对性的措施,控制麻疹发病率。方法选取0～8月龄儿童,采用病例对照研究方法 ,研究病例组和对照组在医院暴露史、麻疹患者接触史、母亲患麻疹史、母亲疫苗接种史和人口信息等可能的影响因素上的差异,从而确定引起宝安区0～8月龄儿童麻疹高发的原因。结果医院暴露和麻疹病例接触OR值分别为4.2、3.5,母亲患麻疹史和母亲疫苗接种史OR值分别为0.1、0.2。叉生分析显示:单独麻疹患者接触史OR值的95%CI为0.87～23.51,而医院暴露史OR值的95%CI为1.27～20.70;院内麻疹患者接触史OR值的95%CI为1.52～83.66。结论医院感染、麻疹病例接触史是宝安区0～8月龄儿童麻疹高发的危险因素,而母亲患麻疹史和母亲疫苗接种史则为保护性因素,提示儿童麻疹疫苗接种月龄可考虑适当前移。
Objective To explore the risk factors of high incidence of measles among children aged below 8 months in Baoan District,and then take the corresponding measures for measles control.Methods A case-control study was carried out among the children aged below 8 months for seeking risk factors,such as hospital exposure history,history of contact with measles cases,mother’s history of measles,mother’s history of vaccination,and population mobility so as to determine the causes of prevalence.Results Nosocomial exposure and contact with measles cases were the risk factors which made this disease spread,and the OR values were 4.2 and 3.5,respectively.Mother’s history of measles and mother’s history of vaccination were the protective factors,and the OR values were 0.1 and 0.2,respectively.Strata analysis showed that history of contact with measles cases,hospital exposure history,and nosocomial exposure to measles virus were the risk factors,and the 95% confidence interval was 0.87～23.51,1.27～20.70,and 1.52～83.66,respectively.Conclusions Nosocomial infection and history of contact with measles cases are the risk factors of high incidence of measles among children aged below 8 months in Baoan District,while mother’s history of measles and mother’s history of vaccination are the protective factors.The results suggest that the time of measles vaccination should be ahead of schedule.
