摘要
为探讨异位妊娠(ectopic pregnancy,EP)的临床早期诊断及发病相关因素,对经手术及病理证实的106例EP进行了回顾性分析。结果表明:(1)EP的误诊率高达17.9％,其误诊原因主要是病史询问不仔细,被带宫内节育器(intrauterine device,IUD)或已绝育而掩盖,误诊为先兆流产、不全流产12例,占误诊63.2％,对早期诊断进行讨论。(2)EP的相关因素有慢性盆腔炎症、盆腔手术史、不孕症,尤其是EP与IUD的关系密切,带IUD发生EP有41例,占EP的38.7％,居相关因素之首。(3)带IUD的EP着床部位多在输卵管的中、远端,与不带IUD的EP发生部位有差异(P<0.05)。(4)用性激素治疗不孕症时要警惕EP的发生。
In order to study the early clinical diagnosis and related factors of causing disease 106 cases of ectopic pregnancy proved by operation and pathology are aralyzed. It shows that; (1) The rate of misdiagnosis is as high as 17.9%. Because of carelessly inquiring the patient' s history and confused by intrauterine device and stevilzation 12 cases of E. P were misdiagnosed as threatened abortion and incomplete abortion, 63. 2% . (2) The related factors of E. P are the following ; Pelvic inflammation, operation history on pelvic and sterility, especially E P. has close contact with IUD. 41 cases of IUD, which is 38.7% , Stand at the first place. (3) Most of the implantation part of E. P with IUD lie in the middle and far end of oviduct. They are different from the E P without IUD. P < 0. 05. (4) Be careful that while treating sterility with go-nadotherapy may lead to E. P.
