摘要
目的比较患儿口服不同剂量琼脂加蓝光治疗与单纯蓝光治疗降低新生儿血清胆红素水平的效果。方法165例新生儿黄疸患儿分为观察Ⅰ组、观察Ⅱ组、对照组。观察I组口服琼脂250mg(kg.次),1次/6h,观察Ⅱ组口服琼脂500mg/(kg.次),1次/6h,两组均加蓝光治疗8h/d;对照组采用常规蓝光治疗,8h/d。比较两观察组之间以及两观察组与对照组之间患儿的血清总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)水平的影响变化。结果两个观察组的患儿在接受治疗后血清TBIL、IBIL水平下降幅度显著高于对照组(P<0.05),但是患儿的直接胆红素(DBIL)水平下降幅度与对照组差异无统计学意义(P>0.05);两个观察组患儿的血清TBIL、DBIL、IBIL水平下降幅度差异无统计学意义(P>0.05)。结论口服琼脂加蓝光治疗对于降低新生儿血清总胆红素和间接胆红素水平的效果显著好于对照组,但是两个观察组患儿TBIL、DBIL、IBIL的下降幅度没有显著差异。口服琼脂安全、方便,经济、有效,推荐采用琼脂剂量为250mg/(kg.次),1次/6h,能达到降低血清胆红素水平的较好效果。
Objective To compare children of different doses of oral agar plus Blu-ray treatment of neonatal serum bilirubin levels and Blu-ray treatment alone to reduce the effect of neonatal serum bilirubin level.Methods 165 cases of neonatal jaundice were divided into o bservation group Ⅰ,Ⅱ observation group,the control group.Observed in group Ⅰ oral agar 250mg/（kg·times）,q6h,observed II group was treated with agar 500mg/（kg·times）,q6h,plus Blu-ray treatment in both groups 8h/day;the control group treated with conventional blue light,8h/day.Comparison of both groups as well as between two observation group and control group children between the serum total bilirubin（TBIL）,direct bilirubin（DBIL）,indirect bilirubin（IBIL） the impact of changes in the level.Results The results of two groups of children observed after treatment of serum TBIL,IBIL levels drop significantly higher（P0.05）,but the children with the direct bilirubin （DBIL） decrease the level of the control group no significant difference（P0.05）;two observed groups of children with serum TBIL,DBIL,IBIL decrease the level of no significant difference（P0.05）.Conclusion Of oral agar plus Blu-ray treatment are to reduce neonatal serum total bilirubin and indirect bilirubin levels significantly better results in the control group,but the two children with the observation group TBIL,DBIL,IBIL decline was not significantly different.Oral agar safe,convenient,economical,effective and recommended agar dose of 250mg/（kg·times）,q6h,can achieve lower serum bilirubin levels of better results.
