摘要
目的观察剖宫产术后24小时内乳头刺激与婴儿吸吮对子宫收缩及阴道出血量的影响,探讨减少产后出血的护理方法。方法选择162例剖宫产术后,无内外科合并症,无严重妊娠合并症,新生儿体重2500～3500g,妊娠次数3次以内的初产妇,分为甲乙两组,甲组85例指导和协助其在术后24小时内给予频繁吸吮和乳头刺激各8次,乙组77例指导和协助其在术后24小时内给予吸吮8次。结果 24小时阴道出血量甲组(68.31±32.65)ml,乙组(91.19±42.62)ml,差异有统计学意义(t=-3.808,P<0.0001)。甲组中58例子宫收缩好,阴道出血量<60ml;24例子宫收缩好,阴道出血量<80ml,3例阴道出血量>80ml;乙组8例子宫收缩好,阴道出血量<60ml,24例子宫收缩好阴道出血量<80ml,45例阴道出血量>80ml。结论剖宫产后24小时内婴儿吸吮并进行乳头刺激,可作为减少产后出血的护理方法之一。
Objective To observe the influence of baby sucking and nipple stimulation in 24 hours after cesarean section upon uterine contraction and the amount of vaginal bleeding to explore the nursing ways to reduce postpartum hemorrhage, nethotis One hundred and sixty-two cases of post-cesarean section without significant surgical complications and pregnant complications were involved in our study. All patients were randomly divided into 2 groups. The 85 patients in group A were guided and assisted to give suck and stimulate nipples more than 8 times during 24 hours after CD, while the 77 patients in group B were guided and assisted to give suck less than 8 times. Results In 24 hours, vaginal bleeding in group A was 68. 31 ＋ 32. 65, while that in group B was 91.19 ±42. 62. The difference was statistically significant （ t = - 3. 808, P 〈 0. 0001 ）. Fifty-eight eases in group A and 8 cases in group B appeared a good uterine contraction. The amount of vaginal bleeding was less than 60ml. Twenty-four eases in both two groups appeared a good uterine contraction. The amount of vaginal bleeding was less than 80ml. The amount of vaginal bleeding was more than 80ml in 3 cases of group A and 45 cases of group B. Conclusion Baby sucking and nipple stimulation in 24 hours after cesarean section is helpful to improve uterine contraction and decrease vaginal bleeding.
